Abstract

Alkylresorcinoler (AR) är en molekylgrupp som finns i hög koncentration i klidelen av råg och vete men inte i betydande mängd i andra spannmål eller vanliga livsmedel. AR och dess metaboliter har föreslagits och utvärderats som biomarkörer för full- kornsintaget av dessa sädesslag. I denna studie har de redan beprövade AR-metabo- literna 3,5-dihydroxibensoesyra (DHBA) och 3-(3,5-dihydroxifenyl)-1-propansyra (DHPPA) kvantifierats i 24 timmars urin från deltagare i en tysk diabetesprevent- ionsstudie. Syftet var att undersöka deltagarnas följsamhet till interventionsstudiens respektive dieter. Dieterna var en lågriskdiet (LR) bl.a. rik på fullkorn och en hög- riskdiet (HR) bl.a. fattig på fullkorn. De nyligen upptäckta metaboliterna 5-(3,5-di- hydroxifenyl)-pentansyra (DHPPTA), 2-(3,5-dihydroxibensamid)-ättiksyra (DHBA- Gly) och 3,5-dihydrocyfenylkanelsyra (DHCA) kvantifierades samtidigt för att un- dersöka deras potential som fullkornsbiomarkörer för vete och råg.
AR-metaboliterna dekonjugerades med β-glucuronidas och sulfatas över natt och extraherades sedan med lösningsmedel (etylacetat). Extrakten renades med fastfa- sextraktion kolonner (OASIS-MAX®) och analyserades med gaskromatografi- masspektrometri.
Resultaten visade att merparten av deltagarna i LR-gruppen hade höga mängder AR metaboliter och att deltagarna i HR-gruppen hade låga mängder efter dieternas start. Mängderna varierade dock kraftigt inom grupperna. DHBA, DHPPA, DHCA och DHBA-Gly kunde uppmätas i nästan samtliga prover, medan DHPPTA återfanns i 81 % av proverna. För DHBA, DHPPA och DHCA var precisionen i analysen god- tagbar medan precisionen för DHPPTA och DHBA-Gly var sämre. DHPPA och DHCA visade stark korrelation till övriga metaboliter, medan DHBA och DHBA- Gly uppvisade medelstarka korrelationer. DHPPTA var svagt korrelerade till övriga metaboliter. Metaboliterna visade liknande beteendemönster över tid. Båda grupper- nas deltagare hade medelhöga metabolitnivåer vid baslinjen för att sedan stiga i LR- gruppen och sjunka i HR-gruppen. Efter dieternas start skiljde sig grupperna signifi- kant (p=0,001) för alla metaboliter. Inga signifikanta skillnader kunde ses mellan kvinnor och män.
Slutsatserna var att flertalet deltagare i DDIET-studien tycktes följa sina dieter av- seende fullkornsintaget, då profilen av AR metaboliter i urinen följde mönstret för respektive diet. Bland de nya metaboliterna var DHCA en god biomarkörkandidat tack vare sin höga detekterbarhet och medelhöga korrelation med biomarkörerna DHBA och DHPPA. DHBA-Gly och DHPPTA var inte lika starka som kandidater då de hade lägre korrelation. Metodoptimering krävs för bättre utvärdering av DHCA, DHPPTA och DHBA-Gly som biomarkörer för fullkornsintag av råg och vete.

Alkylresorcinols (AR) is a molecular group found in high concentration in the bran part of wheat and rye grains, but not in other grains or common foods. AR and its metabolites has therefore been proposed and evaluated as biomarkers for whole grain intake of these grains. In this study, the already proven AR-metabolites 3,5- dihy- droxybenzoic acid (DHBA) and 3-(3,5-dihydroxyphenyl)-1-propanoic acid (DHPPA) have been quantified in 24-hour urine of participants in a German diabetes prevention trial. The aim was to explore the participants' adherence to the diets they were assigned in the intervention trial. One diet was a low-risk diet (LR) rich in whole grains and the other was a high-risk diet (HR) poor in whole grains. The newly dis- covered metabolites 3,5- dihydroxyphenylcinnamic acid (DHCA), 5-(3,5-dihydrox- yphenyl)pentanoic acid (DHPPTA) and 2-(3,5-dihydroxybenzamido)acetic acid (DHBA-Gly) were also quantified simultaneously to investigate their potential as whole grain biomarkers for wheat and rye.
AR-metabolites were deconjugated by β-glucuronidase and sulfatase overnight and then extracted with ethyl acetate. The extracts were purified by solid phase ex- traction (OASIS MAX®) and analysed by gas chromatography-mass spectrometry.
The results suggested that most participants adhered to their diets with regards to the grain intake based on high amounts of AR metabolites in the LR group and low amounts in the HR group during their diets, though there were large variations of amounts within both groups. DHBA, DHPPA, DHCA and DHBA-Gly were quanti- fied in almost all samples whereas DHPPTA was found in 81% of the samples. For DHBA, DHPPA and DHCA the precision of the analysis was acceptable, while the precision of DHPPTA and DHBA-Gly was poor. DHPPA and DHCA showed strong correlation to other metabolites, while the DHBA and DHBA-Gly showed less strong correlations. DHPPTA was weakly correlated to other metabolites. The metabolites showed similar patterns over time. Both groups had average high metabolite levels at baseline, the values then rose for the LR group and decreased the HR group. During the diet the groups differed significantly (p=0.001) in amount for all metabolites. No significant differences were seen between men and women.
The conclusions were that most participants in the study followed their diets since the profile of AR metabolites in urine followed each diet. Among the new metabolites DHCA seemed to be a good biomarker candidate due to the high detectability and relatively good correlation with biomarkers DHBA and DHPPA. DHBA-Gly and DHPPTA were not as good candidates due to the lower correlation. Method optimi- zation is required for further evaluation of DHCA, DHPPTA and DHBA-Gly as bi- omarkers of whole grain intake of rye and wheat.
