Abstract

The aim of this retrospective cross-sectional study was to quantitively study whether the longitudinal rotation of the cervical spine had a significant effect on the intra- and intervertebral ratio on lateral conventional radiography of the cervical spine on horses. The ratios quantify the relation between the sagittal dorsoventral diameter of the vertebral canal and the sagittal dorsoventral height of the
vertebral body. The effect of rotation was studied through digital reconstructions of cross-sectional series with three-dimensional computed tomography to simulate lateral conventional radiography with the degrees of rotation of 0°, 5°, 10° and respectively 15°.
The study was founded on the hypothesis that there would be a significant negative correlation between the degrees of longitudinal rotation in the cervical spine and the intra- and intervertebral ratio. An increase of the degree of rotation was expected to correlate with a decrease of the intraand intervertebral ratio. The study aimed to also quantify the intra- and interobserver agreement for
intra- and intervertebral ratio on simulated lateral conventional radiographs with different degrees
of rotation.
The results revealed how rotation had a significant effect on intra- and intervertebral ratio. A significant difference in the intervertebral ratio could be detected at the degree of rotation of 15°, compared to the degree of rotation of 0°, 5° and respectively 10° (p ≤ 0,0019). A tendency toward
significant difference for intervertebral ratio I, without achieving statistical significance, could be detected between 10° and 15° (p = 0,0660). A significant difference could be detected for intervertebral ratio II between 0° and 15° (p = 0,0197), as well as 5° and 15° (p = 0,0307). Another tendency toward significant difference for intervertebral ratio II, without achieving a level of
significance, could be detected between 10° and 15° (p = 0,0726). The intraobserver agreement exhibited moderate to excellent reliability, while the interobserver agreement exhibited poor to excellent reliability. The intravertebral ratio exhibited the lowest reliability (ICCintra 0,624; ICCinter 0,465) and measurement D the highest reliability (ICCintra 0,953; ICCinter 0,922) for both the intraand the interobserver agreement.
The result of the study indicated that rotation in cervical spine had a significant effect on the intra- and intervertebral ratio at degrees of rotation higher than 10°. There was a significant positive correlation between the degree of rotation and the intra- and intervertebral ratio. An increase in the degree of rotation correlates with an increase in the intra- and intervertebral ratio. The intra- and interobserver agreement suggests that the estimation of intra- and intervertebral ratio could vary both within and between the observers. An increase in the degree of rotation ≥ 10° correlates with an increased risk for inconsistent measurements between the observers. The study’s limitations of the group size indicate that additional studies are required to find further potential conclusions regarding the effect of rotation on the intra- and intervertebral ratio. The results of this study are expected to be an incentive for future research with the potential of strengthening scientific evidence and contributing to clinically relevant implications for imagining diagnostics of cervical spines in
horses within veterinary medicine.
