Abstract

Insulindysreglering (ID) är ett utbrett problem inom hästpopulationen. Hyperinsulinemi i samband med ID utgör den vanligaste orsaken till att hästar utvecklar fång. Humana diabetesläkemedel tillhörande gruppen Natrium-glukos-kotransportör 2 (SGLT2)-hämmare har visat vara effektiva för att sänka insulinkoncentrationen i blodet hos hästar med ID och därmed reducera risken för fång. Natrium-glukos-kotransportör 2-hämmare minskar återupptag av glukos från primärurinen vilket orsakar glukosuri och sekundärt sänkt glukos- och insulinkoncentration i blodet. En minskning av ß-cellernas känslighet för glukos, dvs insulinsekretionen vid en given blodkoncentration, har diskuterats som en bidragande orsak till sänkta insulinnivåer hos ID hästar behandlade med SGLT2-hämmare. I en svensk fältstudie där hästar med ID behandlades med SGLT2-hämmaren kanagliflozin sågs ett bristfälligt behandlingssvar med avseende på insulinnivåer under ett utfodringstest (meal tolerance test, MTT) hos 13 % av de inkluderande hästarna. Det är således intressant att kunna utvärdera behandlingseffekt, framför allt hos hästar som svarat suboptimalt på behandling, via utvärdering av ß-cellsrespons och mätning av glukosförlust via urinen. I dagsläget är detta tekniskt krävande, kostsamt och ej praktiskt genomförbart kliniskt. Syftet med aktuellt studentarbete är därför att utvärdera hur väl enklare tester som kan utföras kliniskt korrelerar med etablerade referensmetoder.

Studentarbetet baserades på två separata studier där totalt 61 privatägda hästar deltog. I den första studien undersöktes 41 hästar med ID med en graded glucos infusion (GGI), en metod där ß-cellsrespons mäts vid intravenös glukosinfusion med ökad infusionshastighet. Hästarna
genomgick också ett oralt sockertest (OST) där glukossirap gavs direkt i munnen och ett utfodringstest MTT där de gavs en standardiserad giva av sitt eget grovfoder. Alla undersökningar utfördes före och efter tre veckors behandling med SGLT2‑hämmaren kanagliflozin (0,6 mg/kg eller 1,2 mg/kg) eller placebo. Den beräknade ß-cellsresponsen från GGI:n jämfördes med kvoten mellan insulin- och glukoskoncentrationen i blodet vid valda tidpunkter under OST och MTT. I den andra studien behandlades 20 hästar med ID, med kanagliflozin i dosen 0,4 mg/kg eller placebo i tre veckor. Hästarna undersöktes med vattenmätning över ett dygn, fasteurinprov där koncentration av glukos och kreatinin analyserades efter en natts fasta, samt urinprov där total volym urin mättes och koncentrationen glukos analyserades i samband med en tre timmar lång euglycemic hyperinsulinemic clamp (EHC). Glukosutsöndringen under EHC:n användes för att kvantifiera glukosutsöndringen i urin över dygnet. Baserat på glukos- och kreatininkoncentrationen i fasteurinprov samt vattenmätning utformades tre markörer som jämfördes med den beräknade glukosförlusten i urinen vid EHC.

Resultaten visade låg korrelation mellan den uträknade ß-cellsresponsen under GGI och insulin/glukos-kvoterna vid OST och MTT. Däremot var det en god korrelation mellan den fastställda glukosförlusten under EHC och de beräknade markörerna baserade på fasteurinprov.

Sammanfattningsvis tyder detta på att markörer beräknade från enstaka blodprovtagningstillfällen under OST eller MTT inte kan användas för utvärdering av ß-cellsresponsen hos hästar. Däremot kan ett enstaka fasteurinprov ge tillförlitlig information om glukosförlust i urin över ett dygn hos hästar behandlade med SGLT2 hämmare. Detta kan vara användbart vid vidare utredning av individer med bristande behandlingssvar och utebliven sänkning av insulinkoncentrationer under SGLT2 behandling.

Insulin dysregulation (ID) is a widespread problem in the equine population. Hyperinsulinemia associated with ID is the most common cause of laminitis in horses. Human antidiabetic medications belonging to the sodium–glucose cotransporter 2 (SGLT2)-inhibitor class have been shown to
effectively reduce insulin concentrations in horses with ID, thereby decreasing the risk of laminitis. Sodium–glucose cotransporter 2-inhibitors block reabsorption of glucose from the primary urine, thereby inducing glucosuria, leading to lowering of glucose- and insulin concentrations in blood. A reduction in the glucose sensitivity of the β-cells, that is, insulin secretion at a given blood glucose concentration, has been discussed as a contributing factor to the decreased insulin levels observed in ID horses treated with SGLT2-inhibitors. In a Swedish field study in which horses with ID were treated with the SGLT2 inhibitor canagliflozin, an inadequate treatment response with respect to insulin concentrations during a meal tolerance test (MTT) was observed in 13% of the included horses. Consequently, it is of interest to evaluate treatment efficacy, particularly in horses with a suboptimal treatment response, by assessing β-cell responsiveness and urinary glucose loss. At present, such evaluation is technically demanding, costly, and not clinically feasible. The aim of the present master thesis was therefore to assess how well simple, clinically applicable tests correlate with established reference methods.

This undergraduate project was based on two separate studies involving a total of 61 privately owned horses. In the first study, 41 horses with ID were examined using a graded glucose infusion (GGI), a method in which β-cell response is assessed during intravenous glucose infusion with progressively increasing infusion rates. The horses also underwent an oral sugar test (OST), in which glucose syrup was administered orally, and an MTT, in which they were given a standardized ration of their own forage. All examinations were performed before and after three weeks of treatment with the SGLT2-inhibitor canagliflozin (0.6 mg/kg or 1.2 mg/kg) or placebo. The calculated β-cell response from the GGI was compared with the ratio between insulin- and glucose concentrations in blood at selected time points during the OST and MTT. In the second study, 20 horses with ID were treated with canagliflozin at a dose of 0.4 mg/kg or placebo for three weeks. The horses were assessed using 24-hour water intake measurements, fasting urine samples in which glucose and creatinine concentrations were analyzed after an overnight fast, and urine collection during a threehour euglycemic hyperinsulinemic clamp (EHC), during which total urine volume was measured and glucose concentration was analyzed. Glucose excretion during the EHC was used to quantify 24-hour urinary glucose excretion. Based on glucose- and creatinine concentrations in fasting urine samples and water intake measurements, three markers were developed and compared with the calculated urinary glucose loss during the EHC.

The results demonstrated a low correlation between the calculated β-cell responsiveness during the GGI and the insulin-to-glucose ratios obtained during the OST and MTT. In contrast, a strong correlation was observed between the quantified glucose loss during the EHC and the calculated
markers based on fasting urine samples.

In summary, these findings indicate that markers derived from single blood sampling during the OST or MTT cannot be used to evaluate β-cell response in horses. However, a single fasting urine sample may provide reliable information on the 24-hour urinary glucose loss in horses treated with SGLT2-inhibitors. This may be useful in the further evaluation of individuals with inadequate treatment response and a lack of reduction in insulin concentrations under treatment with SGLT2-inhibitors.
