Abstract

Uppvaket är ett moment som innebär risker för hästar som genomgår generell anestesi. Ett dåligt
uppvak kan leda till allvarliga skador så som frakturer eller till och med död. Det är därför viktigt
att veta vad som kan påverka uppvaket negativt. Många faktorer räknas upp som kan ha en negativ
påverkan så som ålder och längd av anestesin. Syftet med denna retroperspektiva studie var att
undersöka om något samband kunde identifieras mellan uppvakets kvalitet och syresättningens
kvalitet under anestesin samt om operationens duration påverkade uppvakskvalitén. Uppvakets
kvalitet bedömdes av två studenter med hjälp av inspelningar av hästarna i uppvaket på Universitets
djursjukhuset (UDS) med två olika skalor för att gradera uppvakskvalitén. Journaler och
blodgasrapporter från operationer som utfördes på UDS mellan 26 jan och 28 feb 2024 samlades in
och datan sammanställdes. Totalt registrerades 54 hästar varav 12st exkluderades ur studien av olika
skäl så som avlivning under operation eller bristande kvalitet på det inspelade materialet.
Arbetet visade inte på något samband mellan syresättning under anestesi och kvalitet på uppvak och
ej heller något samband mellan operationens längd och uppvakskvaliteten. Endast 1 häst hade
uppmätt hypoxemi det vill säga partiellt syretryck (PaO2) <7.99 kilopascal (kPa)under operationen
och endast 6st hästar hade en operationstid på över 2.5 timmar.
Bedömningen av uppvakskvaliteten visade en skillnad på gradering mellan studenterna vilket
indikerar en för stor subjektivitet i de använda bedömningsskalorna för uppvaket och belyste
svårigheten att få mätbara och tillförlitliga resultat.
Resultatet påvisar att fler studier behövs för att kunna svara på frågeställningen med en större
studiepopulation och dessutom att mer objektiva metoder för bedömning av uppvakskvalitet är
indicerat.

Recovery is a part of general anesthesia that involves considerable risk. A bad recovery can lead to
serious injury such as fractures or even death and it is therefore important to know what aspects can
affect the recovery negatively. A lot of factors have been identified to influence the quality of the
horse's recovery, such as age of the horse and length of the surgery. Oxygenation during surgery and
its effects on the recovery have not been throughout evaluated/described/investigated earlier. In this
retroperspective study the quality of the recovery was evaluated by two students using two different
scales with the use of recordings of the horses in the recovery stall. Journals and bloodgas results
that were taken during surgery were compiled and compared. The quality of the recovery was then
evaluated in relation to oxygenation during surgery. A total of 54 patients were evaluated of which
12 were excluded due to either euthanasia unrelated to the recovery or due to video quality not being
sufficient for evaluation of the recovery.
Results of this paper didn't show any connections between oxygenation during general anesthesia
and the quality of recovery. There was no connection found between length of surgery and the
recovery either. Only one horse had recorded hypoxemia (Pao2 <7.99 kPa) and only 6 horses had a
surgery time over 2.5 hours.
Large variations in the scoring of recovery in between the different observers indicate that the
subjective nature of the scoring scales used lead to difficulties finding measurable and reliable
results. A more objective scale or method to evaluate the recovery is desired.
The findings in this small retroperspective study shows that more research is needed to be able
to answer the question if the oxygenation during surgery is related to the quality of the recovery and if you by improving the oxygenation during surgery can decrease the high risk of complications
during recovery.
