Abstract

Hypotension är ett vanligt förekommande problem under anestesi av häst. Det finns
flera åtgärder att vidta; minska inhalationsgasen, administrering av vätsketerapi
eller administrering av läkemedel. Målet med åtgärderna är att uppnå ett
medelartärtryck på över 70 mmHg och bibehålla en adekvat kardiovaskulär
cirkulation, för att minimera risken för komplikationer.
Målet med detta arbete är att utföra en litteratursökning samt en journalstudie
för att undersöka förekomst och åtgärder av hypotension. Originalartiklar,
sammanställningsartiklar, facklitteratur och hemsidor har använts i
litteratursökningen för att hämta information och fakta. Journalerna i denna studie
är tagna från ett svenskt hästsjukhus och bestod slutligen av 9 artroskopifall och 13
hästar som genomgick bukkirurgi. Kriterierna för individerna var bland annat att de
vägde över 400kg, var mellan tre och tjugo år gamla, hade en narkoslängd på över
en timme och fyrtiofem minuter samt att det fanns en anestesijournal inlagd i
journalen.
Litteraturen beskriver att administrering av vätsketerapi och dobutamin är de
främst förekommande åtgärderna mot hypotension. Journalstudien visade att de
rekommendationer som beskrevs i litteraturen tillämpades i de aktuella fallen.
Studien visade även att hästarna som genomgick artroskopi hade högre förekomst
av hypotension mot vad hästarna som genomgick bukkirurgi hade. Det som skilde
de två grupperna åt var tidpunkten när hypotension uppstod, hos artroskopihästarna
uppstod tillståndet i starten medan det hos bukkirurgihästarna uppstod i mitten av
anestesin. Detta kan antyda att val av läkemedel vid induktion kan bidra till att
bibehålla ett adekvat blodtryck. Dock orsakas inte hypotension av en enskild faktor
utan flera aspekter spelar roll.

Hypotension is a common complication during anesthesia of the horse. The
measures taken to prevent or treat hypotension can be to lower the concentration of
inhalation anesthetic, increase fluid rate or administrate pharmaceuticals. The line
for hypotension is that the medial artery pressure (MAP) should be over 70 mmHg
to maintain good circulation throughout the cardiovascular system and minimize
post operative complications.
The aim of this paper is to look at the literature regarding the preventative or
treatment measures taken as well as look at a group of journals from a Swedish
equine hospital. The literature used was original articles, review articles, books and
websites. The journals included in this study are 9 arthroscopy cases and 13
laparotomy surgeries. The cases were selected based on the following criteria;
bodyweight over 400kg, between 3-20 years old, the duration of the anesthesia
lasted over one hour and forty-five minutes and each individual had to have an
anesthesia journal.
The literature suggests administration of fluid therapy and dobutamine as the
primary treatment for hypotension. The journal study showed that the
recommended treatment for hypotension is used in clinical situations. The study
also showed that the arthroscopy cases had a higher prevalence of hypotension
compared to the abdominal surgeries. A difference in time when hypotension
occurred was observed, where arthroscopy horses developed hypotension at the
start of the anesthesia and the laparotomy horses developed hypotension in the
middle of the anesthesia. This suggest that the choice of pharmaceuticals used for
induction could contribute to maintaining an adequate blood pressure. However,
hypotension is not only caused by one single factor and all aspects need to be taken
into consideration.
