Abstract

Syftet med studien var att beskriva radiologiska och artroskopiska fynd hos hundar diagnosticerade med fragmenterad medial coronoidprocess (FMCP) och osteochondros (OC) i armbågsleden samt att kartlägga hur många av dessa som hade ett ED-resultat registrerat hos Svenska kennelklubben (SKK). Data samlades in retrospektivt från journaler mellan 2004-2014. Totalt ingick 80 artroskopierade armbågsleder från 59 hundar. Operationsberättelsen i journalen bedömdes i efterhand av en kirurg (ABE) med avseende på eventuella påvisbara utvecklingsrubbningar och broskdefekter i leden. Broskdefekterna bedömdes utifrån en modifierad Outerbridgeskala. Röntgenbilder tagna i samband med det kliniska besöket bedömdes på nytt av en radiolog (KH) med avseende på om patologiska fynd kunde påvisas eller inte. Medianålder vid röntgentillfället var 8,5 månader med en variation mellan 5 och 101 månader. Grad av eventuella periartikulära osteofyter, subtrokleär skleros samt förändringar på mediala coronoidprocessen och humerus mediala kondyl noterades. Chi-square test användes som statistisk analysmetod för att beräkna eventuell skillnad mellan fördelning av graden av periartikulära osteofyter mellan de olika utvecklingsrubbningarna. ED-resultat samlades in från SKK:s avelsdatabas och samtliga hundar var över ett år när SKK:s data samlades in till studien.
Resultatet av studien visade att 6 av 80 (7,5 %) armbågsleder var normala på röntgen. Dessa konstaterades ha FMCP utan lösa fragment vid artroskopi. Totalt hade 19 leder (23,8 %) inte några påvisbara periartikulära osteofyter på röntgen, 13 av dessa hade andra radiologiska förändringar. Subtrokleär skleros påvisades i totalt 56 (70,0 %) av lederna, varav tolv hade subtrokleär skleros som enda radiologiska fynd. FMCP var det vanligaste fyndet vid artroskopi och kunde ses i 56 (70,0 %) armbågsleder. Totalt 71 (88,8 %) av lederna graderades 3 eller högre på den modifierade Outerbridgeskalan. En signifikant skillnad i fördelning av storlek på periartikulära osteofyter kunde ses mellan de olika artroskopifynden. Av de 53 rashundarna i studien hade 30 (56,6 %) inget ED-resultat registrerat hos SKK, 22 av dessa var yngre än tolv månader vid röntgentillfället. Fyra hundar (7,5 %) hade ett preliminärt ED-resultat och 19 (35,8 %) hade ett officiellt ED-resultat. En tredjedel av de officiella resultaten var ED grad 0.
Slutsatsen var att mer än hälften av hundarna med artroskopiskt diagnosticerad FMCP eller OC i studien saknade ett ED-resultat hos SKK. Information angående tidigare armbågsledsoperation rapporterades inte alltid till SKK. Resultaten tyder på att det bör skapas eller ändra rutiner som ser till att diagnosticerade fall av ED rapporteras in till SKK och att medvetenheten och förståelsen om varför ED-screening genomförs måste öka. Mer utbildning av veterinärer, djursjukskötare, uppfödare och djurägare behövs. Ett fåtal hundar i studien hade inte några radiologiska förändringar vilket tyder på att röntgenbilder kan vara normala hos vissa fall av ED. Hundar utan radiologiska fynd med fortsatt klinisk misstanke om ED måste följas upp. Subtrokleär skleros var ett vanligt radiologiskt fynd och ytterligare studier av armbågsleder med och utan ED kan hjälpa till att utreda värdet av dessa radiologiska förändringar för att upptäcka ED. De radiologiska förändringarna i studien kunde inte förutsäga den artroskopiska diagnosen FMCP eller OC.

The objective of the study was to report radiographic and arthroscopic findings in dogs diagnosed with fragmented medial coronoid process (FMCP) and osteochondrosis (OC) in the elbow joint. A second objective was to determine how many of these dogs that had an ED result registered by the Swedish Kennel Club (SKC). Medical records and radiographs were retrieved from 59 dogs (80 elbows) with elbow dysplasia based on arthroscopic findings between 2004-2014. The surgery records were assessed by a surgeon (ABE) in order to evaluate hereditary joint diseases and cartilage damage, graded by a modified Outerbridge score. Radiographs taken during clinical evaluation were reviewed by a radiologist (KH). Mean age when radiographs were taken was 8,5 months, with a range between 5 and 101 months. Radiographic interpretation included possible pathological findings, grading of periarticular osteophytes, subtrochlear sclerosis and alterations in the appearance of the medial coronoid process and the medial condyle of the humerus. A Chi-square test was used as statistic analysis method to evaluate if any difference could be seen regarding grade of periarticular osteophyte formation and type of hereditary joint disease. ED results were collected from the database of SKC and all dogs were over one year old when the data from SKC were collected.
Six out of 80 elbow joints (7,5 %) were considered radiographically normal, but FMCP without loose fragments was seen in these joints during arthroscopy. A total of 19 joints (23,8 %) did not have any detectable periarticular osteophytes when radiographed, 13 of these had other radiographic findings. Subtrochlear sclerosis was evident in 56 (70,0 %) of the elbow joints, twelve of them had subtrochlear sclerosis as the only radiographic finding. FMCP was the most common arthroscopic finding and could be seen in 56 (70,0 %) elbow joints. Totally 71 (88,8 %) of the joints were graded 3 or higher on the modified Outerbridge score. There was a significant difference in the distribution of size of radiographic osteophytes between the hereditary diseases. Of the 53 pure-bred dogs 30 (56,6 %) did not have an official ED result by SKC, 22 of them were younger than twelve months when radiographed. Four dogs (7,5 %) had a preliminary ED result and 19 (35,8 %) had an official ED result. One third of the official results were ED grade 0.
In conclusion, more than half of the dogs diagnosed with FMCP or OC in the study did not have an ED result by SKC. Information about previous elbow arthroscopy was not always reported to SKC. It seems there is a need to create routines to make sure that dogs with ED based on arthoscopic findings are reported to SKC database. It is important to improve the awareness and understanding of the goals of and reasons for the ED screening program. A few dogs participating in the study had a normal radiographic appearance of the elbow joint which indicates that radiographs can be normal in some cases of ED. Dogs without radiographic findings but with persistent lameness must have a clinical follow up. Subtrochlear sclerosis is a frequently radiographic finding associated with ED, yet it is difficult to evaluate. Further studies of elbow joints with and without subtrochlear sclerosis could help evaluate the importance of this radiographic change for the diagnosis of ED.
