Abstract

Bullous pemphigoid är en autoimmun hudsjukdom som orsakar separation av epidermis och
dermis vilket leder till en subepidermal blåsbildning och drabbade individer får även rodnader
och nässelutslag. Sjukdomen är den vanligaste autoimmuna subepidermala blåsdermatosen
hos människa, men den finns även beskriven hos hund, katt, gris och häst. Den
immunologiska mekanismen bakom bullous pemphigoid är inte helt utredd, men det har
gjorts en hel del forskning för att kartlägga den. Även om sjukdomen är betydligt mycket
vanligare hos människa än hos djur är det av stort intresse att uppmärksamma homologa
varianter av sjukdomen hos djur för att kunna samla in mer information om mekanismerna
bakom sjukdomen. En helt kartlagd sjukdomsmekanism skulle öka förutsättningarna för att
lyckas hitta ett optimalt läkemedel.
Det finns i dagsläget flera olika teorier om vilka komponenter i immunförsvaret som är
viktiga i utvecklingen av blåsbildningen. De olika forskargrupperna har fått olika resultat och
på så sätt genererat motsägande teorier. Majoriteten av de studier som utförts bygger på
experimentella försök på möss. Syftet med denna litteraturstudie är att sammanställa den
befintliga information som finns om den immunologiska sjukdomsmekanismen bakom
bullous pemphigoid.
Sjukdomen uppkommer genom att autoantikroppar bildas mot keratinocyternas
hemidesmosomer. Hemidesmosomens fysiologiska funktion i huden är att länka samman
keratinocyterna i epidermis med strukturer i basalmembranet. Vid uppkomsten av
autoantikropparna separeras därför epidermis från dermis och blåsor i huden bildas.
En klassisk teori om hur antikropparna kan orsaka blåsbildning bygger på att
komplementfaktorer, mastceller och neutrofiler ansamlas och aktiveras i huden. Ett samspel
mellan dessa komponenter leder då till nedbrytning av hemidesmosomen. Det finns dock
nyare studier som tyder på att bullous pemphigoid kan utvecklas utan att komplementsystemet
aktiveras, där antikropparnas påverkan på keratinocyterna direkt kan leda till nedbrytning av
hemidesmosomen. Det diskuteras även om vilken typ av antikropp som är viktigast för
sjukdomsutvecklingen där de äldre studierna fokuserar på IgG-antikroppar medan nyare
studier lyfter fram IgE-antikropparnas roll. Forskningen behöver i framtiden fokusera på att
studera de olika mekanismernas samverkan istället för att försöka bevisa att en av dem är rätt.
Bullous pemphigoid är relativt ovanlig hos hund. De diagnostiska verktygen som krävs för att
fullständigt kunna konstatera fall av sjukdomen är dock begränsade på djursidan och därför
skulle sjukdomen kunna vara kraftigt underdiagnostiserad. För att kunna studera denna
spontant uppkomna djurmodell av sjukdomen, som på humansidan faktiskt är ett stort
problem, bör veterinärer vara uppmärksamma på eventuella fall av bullous pemphigoid.

Bullous pemphigoid is an autoimmune skin disease leading to separation of the epidermisdermis-
junction. The separation results in a subepidermal blister formation and the patients
with this disease also show clinical features as erythema and urticaria. Bullous pemphigoid is
the most common autoimmune subepidermal blistering disease in humans, but it can also
affect dogs, cats, horses and pigs. The immunological mechanisms causing the disease are not
fully understood, even though a lot of research has been done. Even if bullous pemphigoid is
much more common in humans compared to other animal species, it is of interest to observe
animal homologues of bullous pemphigoid in humans. This might provide information about
the immunological mechanisms and thus opportunities to develop the most optimal therapies.
The different studies have resulted in different theories about the components of the immune
system being essential for the blister formation. Most of these studies are based on
experimental methods using mice. This review will summarise the current knowledge about
the immunological mechanisms of bullous pemphigoid.
This autoimmune disease is caused by the production of autoantibodies against the
hemidesmosomes of the keratinocytes. The physiological function of the hemidesmosomes in
the skin is to form a junction between the keratinocytes and the basement membrane. The
autoantibodies against the hemidesmosomes lead to the separation of the epidermis-dermisjunction.
The classical theory about how these antibodies can cause blister formation is based upon the
infiltration and activation of complement, mast cells and neutrophils. Together, these
components are thought to destruct the hemidesmosomes. Conversly, some studies have
shown that bullous pemphigoid can develop without the complement system. These studies
suggest that the autoantibodies can stimulate the keratinocytes to degrade their own
hemidesmosomes. There are also discussions about the different types of antibodies and their
pathogenicity in bullous pemphigoid. The earlier studies focus on IgG-antibodies while recent
studies emphasize the role of IgE-antibodies. In future studies, the researchers need to have all
of these different theories in mind to be able to understand their combined effects.
Bullous pemphigoid is a relatively uncommon disease in dogs. There are limitations in the
availability of the necessary diagnostic tools though, suggesting that bullous pemphigoid
could be more common than what is reported. Veterinarians ought to be aware of the disease
since it provides a spontaneously arising model of the homologous bullous pemphigoid in
humans.
