Abstract

Det här arbetet är utfört i syfte att besvara frågor som; vad är primärt glaukom, vilka mekanismer leder till tillståndet och hur kan vi använda oss av kunskapen för att förbättra hundhälsan?
Primära glaukom hos hund är en bred grupp ögonsjukdomar med ett gemensamt slutstadium och idiopatisk etiologi. Typiskt för tillståndet är skador på retinala ganglieceller och synnerven och följden blir med tiden oftast blindhet. Glaukom diagnosticeras ofta genom mätning av det intraokulära trycket (IOP), då sjukdomen karaktäriseras av en tryckstegring. Tryckstegringen kan teoretiskt sett uppkomma genom ökad produktion eller minskat dränage av kammarvätska, varav det senare är det som anses förekomma hos hund. Andra symtom som kan ses är buftalmi, mydriasis, korneaödem, episkleral kärlinjektion och subluxerad lins.
I övrigt finns dock stora skillnader mellan olika typer av primära glaukom och de delas idag in i öppenvinkelglaukom (POAG), trångvinkelglaukom (PACG) och glaukom med pektinatligamentdysplasi (PLD). PACG och POAG kan särskiljas genom bedömning av bredden av kammarvinkeln och har olika sjukdomsförlopp, där det tidigare är snabbare progressivt än det senare. PLD innebär att onormal vävnad i kammarvinkeln ger en mekanisk obstruktion som försvårar kammarvätskans utflöde, men den fullständiga kopplingen mellan PLD och glaukom är ännu inte klarlagd.
De utförligaste studierna är gjorda på POAG-drabbade beaglar som en modell för den vanligaste formen av humana glaukom, medan PACG anses vara den vanligaste varianten hos hund. Hos beaglar har man bland annat kunnat koppla sjukdomen till mutationer i en gen som kodar för glykoproteiner som finns i det extracellulära matrixet i kammarvätskans utflödesvägar. För PACG är mindre sjukdomsgenetisk forskning gjord, men en studie föreslår att ett defekt muskelprotein (nebulin) skulle kunna påverka stabiliteten av ciliarklyftan. PLD har hos många raser kunnat associeras med glaukom, men förändringen förekommer även hos friska individer, vilket indikerar att utveckling av glaukom vid PLD beror på flera olika faktorer, eventuellt bara delvis genetiska.
Förekomsten av olika typer av glaukom försvårar arbetet med att finna sjukdomsframkallande gener hos predisponerade raser, något som annars är till hjälp i både behandlings- och avelssyfte. För att utföra en genetisk studie av primära glaukom med framgångsrika resultat, bör stor vikt läggas vid tydliga inklusionskriterier och tillräckliga kliniska undersökningar, eftersom det annars finns stor risk för ihopblandning mellan olika former av glaukom eller mellan sjuka och friska hundar.

The purpose of this study is to answer the following questions: what is primary glaucoma, which mechanisms underlie the condition and how can we use this knowledge to improve the health of dogs?
Primary glaucoma in dogs includes a large group of ocular diseases that share a common end stage and idiopathic etiology. Damage to retinal ganglion cells and the optic nerve is typical for the condition and will usually result in blindness with time. Glaucoma is frequently diagnosed by assessing the intraocular pressure (IOP), since the disease is characterized by an increased pressure. This can theoretically be the result of either increased production or decreased outflow of aqueous humor, the latter being the most relevant in dogs. Other clinical signs include buphtalmia, mydriasis, corneal edema, episcleral congestion and a subluxated lens.
There are major differences between different types of primary glaucoma and the glaucomas are usually divided into open angle glaucoma (POAG), angle-closure glaucoma (PACG) and glaucoma associated with pectinate ligament dysplasia (PLD). POAG and PACG can be differentiated by estimating the width of the iridocorneal angle and the progression of the disease is much more rapid in the latter than in the former. PLD is a tissue defect that is believed to result in mechanical obstruction of the outflow pathways, but the relationship between PLD and glaucoma is still not fully understood.
Research in depth within this subject has been done in Beagles with POAG, an animal model for the most common form of human glaucoma, though the most common type of the disease in dogs is PACG. In Beagles, primary glaucoma has been associated with mutations in genes coding for glycoproteins that are expressed in the extracellular matrix within structures in the aqueous outflow pathways of the eye. Little genetic research has been done on PACG in dogs so far, but one study suggests that a defective muscular protein (nebulin) could affect the stability of the ciliary cleft, an important part of the outflow pathways. PLD has been associated with primary glaucoma in many breeds but is also prevalently seen in healthy individuals, which suggests that the development of glaucoma in dogs with PLD includes more risk factors, some of them not necessarily genetic.
The presence of different types of glaucoma complicates the task of finding disease-linked genes in predisposed breeds, which otherwise is of great benefit for improving both treatments and breeding programs. To be able to perform a successful genetic study of a certain form of primary glaucoma, clear and concise inclusion criteria and sufficient clinical data are required to avoid accidentally mixing different forms of glaucoma or healthy and affected dogs.
