Abstract

Osteoartrit (OA) och smärta i samband med detta sjukdomstillstånd är ett stort problem hos hästar och andra djurslag. OA karakteriseras av broskdegeneration, skleros av subkondralt ben, osteofytbildning, varierande grad av inflammation och periartikulär vävnadsfibros hos den drabbade individen.

Sjukdomsprocesserna som ger upphov till smärta vid OA är ej fullt klarlagda och det saknas idag fullgod behandling. Inflammation är en viktig del i processen och proinflammatoriska cytokiner så som interleukin (IL-)1β anses ha en viktig roll i sjukdomsuppkomsten, bland annat på grund av sin stimulering av brosknedbrytande enzymer. IL-1β reglerar också uttrycket av nerve growth factor (NGF) som visats ha en viktig roll i kroniska smärttillstånd vid OA.

Denna studie syftade till att, som i ett steg i kartläggningen av smärtmekanismer vid OA hos häst, undersöka och jämföra förekomsten av NGF och IL-1β i normala och OA-affekterade leder (OAleder).

I studien användes ledbrosk och ledkapsel-preparat från 6 normala och 7 OA-leder som samlats in på slakteri och vid obduktion. Dessa studerades med hjälp av antikroppar riktade mot humant NGF och equint IL-1β. Förekomsten av NGF och IL-1β visualiserades med immunofluorescensmikroskopi och
jämfördes mellan normala och OA-leder avseende antal celler och lokalisation i ledkapsel och ledbrosk. Jämförelsen skedde subjektivt och definierades som lindrig, måttlig eller kraftig.

Immunofluorescensuttrycket avseende IL-1β var lindrigt i både ledkapselvävnad från normala och OA-leder och lokaliserat framför allt i nära association med kärl i den fibrösa delen av kapseln. Ingen skillnad i förekomst av IL-1β kunde ses mellan normala och OA-leder. Ledbroskets kondrocyter visade ingen infärgning av IL-1β i normala eller OA-leder. NGF förekom i ledkapseln i lindrig till måttlig grad i normala leder, framför allt associerat till synoviocyterna som linjerar villi i synovialmembranet. OA-leder visade en tydligt ökad immunofluorescens avseende NGF i ledkapseln jämfört med normala leder. I ledbrosket sågs en måttlig till kraftig förekomst av NGF i kondrocyternas cytoplasma, med likartad förekomst i normala och OA-leder.

Osteoarthritis (OA), associated with pain and subsequent lameness, is a major problem in horses and one of the main reasons for failure to perform. OA is characterized by articular cartilage degeneration, subchondral bone sclerosis, osteophyte formation and varying degrees of inflammation and periarticular tissue fibrosis. The pathogenesis of OA-associated pain is not fully understood and there is currently no satisfactory treatment.

Inflammation is a key part of the OA process and the
proinflammatory cytokine interleukin (IL-) 1β is considered of major importance and responsible for stimulation of catabolic enzymes causing cartilage degradation. IL-1β also regulates the expression of nerve growth factor (NGF), which plays an important role in the pain related to chronic OA joints.

The aim of the study was to delineate and compare the presence of NGF and IL-1β in the articular cartilage and synovial membrane from normal and OA affected equine joints.
The material consisted of articular cartilage and joint capsule tissues from six normal and seven OA joints collected at slaughter or at autopsy. Paraffin embedded tissues were studied using immunofluorescence in combination with antibodies against human NGF and equine IL-1β. Presence of NGF and IL-1β were examined and a comparison between normal and OA joints regarding amount
of positive cells and the localization within the tissues was done. The immunofluorescence was subjectively classified as minor, moderate or marked.

The immunofluorescence of IL-1β was minor in the joint capsule of both normal and OA joints. When present, it was found in the fibrous layer, often closely associated with blood vessels. There was no difference in the immunofluorescence of IL-1β in the capsule of normal and OA joint. The chondrocytes of the articular cartilage did not show any staining of IL-1β in normal or OA joints.

Immunostaining for NGF in the joint capsule was minor to moderate in normal joints, primarily localized to the synovicytes outlining the villi in the synovial membrane. However, the capsule of the OA joints showed marked immunofluorescence suggesting clear presence of NGF, not seen in normal joints. Chondrocytes of the articular cartilage from both normal and OA joints showed moderate to
marked cytoplasmic staining for NGF, without differences detected between amount of cells or location within the cartilage layers.
