Abstract

P-glykoprotein (P-gp) är en effluxtransportör som ursprungligen uppmärksammades för sin kapacitet att skydda tumörceller från flertalet läkemedel. Det har sedan visat sig att P-gp även finns i andra vävnader, som tarm, njurtubuli, lever samt i hjärnans kapillära endotelceller. P-gp är en viktig komponent i blod-hjärnbarriären där den hindrar passage av icke-essentiella substanser till hjärnan. Genen som kodar för P-gp kallas MDR1. Mutationen som flertalet hundraser har är en deletion i det fjärde kodande exonet i denna gen. Mutationen ger upphov till ett icke-funktionellt P-gp, vilket leder till att uttransporten av intracellulära ämnen inte längre kan ske som normalt. Detta ger ett ökat upptag av läkemedel över tarmslemhinnan och minskad eliminering via njurar och lever. Införseln till hjärnan ökar, vilket kan ge neurologisk toxicitet.
Denna litteraturstudie inriktar sig på att besvara frågeställningen kring vad mutationen har för fysiologisk påverkan, vilka hundraser som drabbas, hur mutationen påverkar behandling med makrocykliska laktoner samt vilka parasiter som behandlas med makrocykliska laktoner.
Makrocykliska laktoner används för behandling av flertalet nematoder och ektoparasiter hos hund, bland annat Toxocara canis, Pneumonyssoides caninum, Dirofilaria immitis och Demodex canis. Makrocykliska laktoner stimulerar parasiternas inhibitoriska GABAA-kanaler och glutamatreglerade kloridjonkanaler. Detta ger en ökad permeabilitet för kloridjoner över cellmembranen, vilket leder till hindrad synapsöverföring och paralys av parasiten. Preparaten är säkra för värddjuret bland annat eftersom däggdjur saknar glutamatreglerade kloridkanaler och endast har GABA-känsliga neuron i CNS, som skyddas av blod-hjärnbarriären. Hos hundar med MDR1-mutationen kan makrocykliska laktoner obehindrat ta sig in till hjärnan och där utöva samma verkningsmekanism som hos parasiten. Detta leder till ataxi och CNS-depression hos hunden.
Att som veterinär förlita sig till tänkespråket ”white feet don’t treat” i beslutet gällande behandling med makrocykliska laktoner är inte hållbart, eftersom de drabbade raserna har olika utseende samt att även många blandrashundar bär på mutationen. Att helt utesluta behandling av drabbade raser med vissa preparat är inte heller optimalt eftersom den kanske effektivaste behandlingen mot vissa åkommor då elimineras. Mer rutinmässig MDR1-genotypning skulle göra det möjligt att använda sig av individualiserad behandling med P-gp-substrat. På så vis skulle ett mer optimerat behandlingsresultat kunna fås. Det allra bästa vore att få fram ett snabbtest avsett för användning på klinik, så att veterinären direkt på plats kan få reda på om hunden bär på mutationen eller inte.

P-glycoprotein (P-gp) functions as an efflux pump and transport a diverse range of substances over the cell membrane, which leads to decreasing intracellular concentration. P-gp was originally identified by its ability to exert multidrug resistance in tumor cells. Apart from tumor cells, P-gp is expressed in a variety of normal tissues, such as the intestines, renal tubular cells, liver cells and brain capillary endothelial cells. In the blood-brain barrier P-gp is an important component to limit entry of xenobiotics. P-gp is encoded by the MDR1 gene. A deletion mutation exists in the fourth encoding exon in the MDR1 gene of many dog breeds. This mutation results in a non-functional P-gp, leading to defect substrate efflux. A defect P-gp results in enhanced enteral drug absorption as well as reduced urinary and biliary drug elimination. Permeation of xenobiotics to the brain is also increased, which can cause adverse neurological effects.
The aim of this study is to explain the physiological impact of the mutation, which breeds are affected, impact of the mutation on treatment with macrocyclic lactones and examples of parasites that are normally treated with macrocyclic lactones.
Macrocyclic lactones are widely used for the treatment of nematodes and ectoparasites, in dogs particularly against Toxocara canis, Pneumonyssoides caninum, Dirofilaria immitis and Demodex canis. Macrocyclic lactones bind to the inhibitory GABAA-gated chloride channels and glutamate-gated chloride channels of the parasites. This causes increased influx of chloride ions into the neurons leading to impaired synapse transmission and paralysis of the parasite. Glutamate-gated chloride channels are specific to invertebrates and GABAA receptors are only present in the CNS of vertebrates. These two factors contribute to the safety of macrocyclic lactones in the host. In dogs with the MDR1 mutation the macrocyclic lactones will enter the brain in high concentrations and exert the same mechanism as in the parasite. The dog will experience ataxia and CNS depression.
The adage “white feet, do not treat” can not be applied in the decision whether to treat a dog with macrocyclic lactones or not. Due to that affected breeds differ in appearance, and that the mutation is also found in crossbreeds. Since it possibly eliminates the best treatment of some disorders, it is not ultimate to completely exclude these substances in the affected breeds. A DNA-based test to detect the MDR1 mutation will allow a more individualized pharmacological therapy. In this way therapy would be optimized. A quick test intended for clinical use would allow the veterinarian to discover carriers of the mutation straight away.
