Abstract

Fluorokinoloner är viktiga för behandling av bakteriella infektioner inom både humanmedicinen och veterinärmedicinen. De används speciellt vid komplicerade och akuta infektioner då de har ett brett spektrum, baktericid effekt och en stor distributionsvolym. Den ökande resistensen som utvecklas mot fluorokinoloner är ett hot mot både enskilda individer och samhället som helhet. I denna litteraturstudie undersöks fluorokinoloner och olika strategier att hindra resistensuppkomsten mot dem.
Resistensen mot fluorokinoloner sker främst genom två mekanismer, men ytterligare en mekanism har nyligen upptäckts. De två främsta mekanismerna är förändring av målenzymen och minskad mängd fluorokinoloner i bakterien. Minskad mängd fluorokinolon i bakterien erhålls genom minskat intag samtidigt som uttransporten ökar. Den tredje resistensmekanismen är plasmidmedierad och verkar genom protein som skyddar målenzymen från fluorokinoloner. Ett första steg i att minimera utvecklingen av resistens mot fluorokinoloner är att kliniker och sjukhus använder sig av good practice. Detta genom riktlinjer gällande både desinficering av utrustning och miljö, ordentlig handhygien samt riktig och restriktiv användning av fluorokinoloner. Grundläggande åtgärder som dessa har används länge och varit en stor hjälp för att minska risken för resistensuppkomst. Risken måste dock sänkas ännu mer med hjälp av nya strategier.
Många studier har utförts och pågår för att hitta nya metoder som kan hindra uppkomsten av resistens mot fluorokinoloner. Rätt dosering av fluorokinoloner för att undvika resistensuppkomst är en ny metod som i nuläget utreds. Variabeln minimum prevention concentration (MPC) har upptäckts. Resistensrisken minskar om koncentrationen av fluorokinolon i vävnaderna och plasman överstiger MPC. Ytterligare en ny metod som utreds är användning av bakteriofager som komplement eller alternativ till fluorokinolonbehandling. Dessa virus angriper bakterier och kan i kombinationsbehandling öka fluorokinolonernas effektivitet vilket minskar resistensutvecklingen. Möjligheten till utveckling av vacciner som kan höja bakteriernas känslighet mot fluorokinoloner undersöks också. Vaccinet riktas då mot resistenta stammar inom bakteriearten vilket gör att de inte längre har en evolutionär fördel jämfört med icke-resistenta stammar.
Både nya vaccin, bakteriofager som komplementbehandling och rätt dosering är lovande metoder för att minska resistensuppkomsten. Fler experimentella och kliniska studier behövs emellertid innan dessa behandlingsmetoder kan användas rutinmässigt inom vården. Även om fokus måste läggas på att utveckla nya metoder får inte de grundläggande åtgärderna glömmas bort. Genom att alltid implementera good practice minskar resistensuppkomsten och spridningen av resistens. För att minimera risken för resistens mot både fluorokinoloner och andra antibiotikagrupper bör de grundläggande åtgärderna och nya metoder kombineras inom både humanmedicinen och veterinärmedicinen.

Fluoroquinolones are important in the treatment of bacterial infections in both human medicine and veterinary medicine. They are used especially in complex and acute infections because they have a broad spectrum, a bactericidal effect and a large volume of distribution. The increasing development of resistance against fluoroquinolones is a threat to both individuals and the society at large. In this study both fluoroquinolones and different strategies to prevent the development of resistance against them are investigated.
Resistance to fluoroquinolones occurs mainly through two mechanisms, but an additional mechanism has recently been discovered. The two main mechanisms are mutations of the target enzymes and decreased amount of fluoroquinolones in the bacteria. Reduced amount of fluoroquinolones in the bacteria is obtained through reduced intake while the efflux increases. The third mechanism of resistance is mediated by plasmids and acts through proteins that protect the enzyme targets of fluoroquinolones. A first step in minimizing the development of resistance to fluoroquinolones is implementing the use of good practice in clinics and hospitals. This goal is reached through guidance regarding both the disinfection of equipment and the environment and proper hand hygiene as well as proper and restrictive use of fluoroquinolones. Basic actions such as these have been used for a long time and shown to be a big help in reducing the development of resistance. New strategies must be used to reduce this risk additionally.
Many studies have been conducted and are being conducted to find new methods that may prevent the emergence of resistance to fluoroquinolones. Using correct dosage of fluoroquinolones in order to avoid resistance is currently being investigated. The variable minimum prevention concentration (MPC) has been detected. Emergence of resistance is reduced if the concentration of fluoroquinolone in tissues and plasma exceeds the MPC. An additional new approach is being investigated. This is using bacteriophages as a complement or alternative to treatment with fluoroquinolones. These viruses attack the bacteria and can be combined in treatment with fluoroquinolones to increase the efficiency, which reduces the emergence of resistance. The possibility of developing vaccines that may increase susceptibility of bacteria to fluoroquinolones is also being examined. These new vaccines are directed against resistant strains of bacterial species, which means that they no longer have an evolutionary advantage over non-resistant strains.
Both new vaccines, bacteriophages as a complement treatment and the use of right dosage are promising methods to reduce the development of resistance. However, more experimental and clinical studies are needed before these therapies can be used routinely in health care. Although the focus must be on developing new methods, it is important that basic measures are not forgotten. By always implementing good practice the emergence and spread of resistance is reduced. To minimize the risk of resistance against both fluoroquinolones and other antibiotics basic measures should be combined with new methods in both human medicine and veterinary medicine.
