Abstract

Uveit är en vanlig ögonsjukdom hos häst som kan orsaka kroniska förändringar i ögat samt synbortfall. Etiologier kan vara trauma, systemiska infektioner eller neoplasi. En speciell form av uveit är kronisk exacerbativ uveit (Equine recurrent uveitis, ERU) vilken karaktäriseras av återkommande skov med inflammation och anses vara en immunmedierad sjukdom. Behandlingen av uveit hos häst är i första hand medicinsk med atropin topikalt, kortikosteroider topikalt samt NSAID topikalt eller systemiskt samt även behandling av eventuell primärorsak. Atropin är en substans med mydriatiska och cykloplegiska effekter som bidrar till smärtlindring, minskad risk för synechier i ögat samt minskat läckage av proteiner och celler över inflammerade kärl i iris. Atropin i mycket höga doser topikalt har visats ge minskad tarmmotilitet och kolik (Williams et al., 2000). I vår experimentella studie undersöktes effekten på magtarmkanalen vid en dosering av topikalt atropin som är rekommenderad (Hollingsworth, 2011) och vanligt förekommande vid Hästkliniken, Universitetsdjursjukhuset, (totalt 11 mg atropin under fem dygn) vid uveitbehandling. Vid låg dos atropinbehandling kunde nedsatta tarmljud auskulteras vid fler undersökningstillfällen än vid placebo, 38 % respektive 32 %. En behandlingseffekt kunde dock inte verifieras statistiskt (p=0,17). Ingen häst visade tecken på kolik eller påverkat allmäntillstånd under försöket. Slutsatsen är därför att den studerade atropindosen kan användas med något mindre oro än tidigare trott men då studien är liten och alla faktorer som påverkar tarmmotiliteten inte studerades så bör veterinär och djurägare vara fortsatt observanta på koliksymtom vid uveitbehandling.

Uveitis is a common ocular disease in horses that can cause chronic changes in the eye and loss of vision. Potential causes are trauma, systemic infections and neoplasia. A special form of uveitis is equine recurrent uveitis, ERU, which is characterized by recurrent episodes of uveitis and is thought to be an immune-mediated disease. The treatment of uveitis in horses is mainly medical with topical atropine, topical corticosteroides and NSAID topically or systemically. Atropine is a substance with mydriatic and cycloplegic effects, which are important for analgesia, to decrease the risk for synechia in the eye and to decrease the leakage of proteins and cells over inflamed vessels in iris. Very high doses of atropine topically have been shown to decrease gut motility and induce colic (Williams et al., 2000). In our experimental study, the effect on gut motility of recommended (Hollingsworth, 2011) and frequently used doses at Hästkliniken, Universitetsdjursjukhuset, for topical atropine (totally 11 mg during five days) were studied. 38% of the horses subjected to low dose atropine and 32% of the horses subjected to placebo were, through auscultation, found to have decreased gut sounds, but the difference between treatments were not statistically verified (p= 0,17). No horse showed signs of colic or impaired general condition. The conclusion is therefore that the dose of atropine employed in this study can be used with a little less concern. However, the study included few horses and all factors that influence gut motility were not studied. It is therefore recommended that the veterinarian and the owner should continue to be observant of signs of colic during treatment of uveitis.
