Abstract

C-reaktivt protein (CRP) är ett akutfasprotein som ökar snabbt och kraftigt vid inflammationstillstånd och vars nivå korrelerar väl med inflammationstillståndets omfattning och allvarlighetsgrad.

Syftet med denna studie var att undersöka förändring av CRP-koncentration i serum hos hund i samband med kirurgi, om association kunde ses mellan vissa sorters operationer och vissa CRP-nivåer, samt om skillnad i CRP-nivå kunde ses mellan hundar med och utan postoperativa tecken på sjukdom som inte är att förvänta efter kirurgi. Avsikten är att hjälpa veterinärer vid bedömning om en operation orsakat en förväntad omfattning av vävnadsskada samt ifall en avvikande CRP-nivå kan användas som hjälp för att identifiera patienter med postoperativa komplikationer.
Blodprover från totalt 58 hundar som genomgick olika sorters kirurgiska ingrepp analyserades med en immunoturbidometrisk metod. De flesta hundar med låg koncentration av CRP innan operation hade en ökad koncentration 24 timmar efter operation (n=44), men i två fall där hundarna genomgått kirurgi med liten vävnadstraumastorlek (artroskopi och orkiektomi) sågs ingen ökning i CRP-koncentration 24 timmar efter operation.
Av hundarna som provtogs både 24 och 48 timmar efter operation hade fem hundar högst koncentration 24 timmar efter operation och fem hundar hade högst koncentration 48 timmar efter operation. Det vore av värde med ytterligare studier för att undersöka mer exakt hur lång tid efter ett kirurgiskt ingrepp man kan förvänta sig högst koncentration CRP.

CRP-koncentrationerna 24 timmar efter operation, för hundarna som hade CRP-koncentrationer <30 mg/l preoperativt, varierade från 37 till 150 mg/l efter osteotomi (n=9), från normal nivå (<5 mg/l) till 194 mg/l efter artroskopi eller artrotomi (n=8), från 12 till 69 mg/l efter kirurgisk stabilisering av fraktur/-er (n=4), från normal nivå (<5 mg/l) till 102 mg/l efter orkiektomi (n=6), från 58 till 80 mg/l efter ovariohysterektomi utan livmodersjukdom (n=5) och från 38 till 165 mg/l efter ovariohysterektomi med livmodersjukdom (n=14). Den stora variationen i CRP-koncentration inom varje grupp tyder på att stora individuella skillnader i CRP-svar på kirurgi finns och att andra faktorer än typ av kirurgiskt ingrepp har betydelse för CRP-koncentrationen dagen efter operation.
Inget samband sågs i denna studie mellan postoperativa tecken på sjukdom och avvikande CRP-värden jämfört med övriga hundar som genomgått samma typ av kirurgi. Det var dock få hundar per grupp och typer av postoperativa tecken på sjukdom varierade mycket, vilket gör att det inte går att dra några slutsatser av detta. Ytterligare studier av CRP vid postoperativa komplikationer vore av värde.

C-reactive protein (CRP) is an acute phase protein, which concentration in serum is characterized by a rapid and distinct increase as response to inflammatory stimulus, and which correlates well to the extent and severity of the inflammatory condition.

The purpose of this study was to investigate the changes in CRP concentration in dogs with surgical traumas, if certain types of surgical procedures could be associated with certain levels of CRP, and if difference in CRP levels could be seen in dogs with and without clinical signs postoperatively that are not expected after surgery. The intention is to help veterinarians interpret whether an operation caused an expected amount of tissue damage and if an unusual CRP response can be of help to identify patients with complications from the surgery.

The serum levels of CRP from 58 dogs undergoing various surgical procedures were measured by a turbidometric immunoassay. Most of the dogs with a normal level of CRP before surgery had an increase in CRP concentration 24 hours after surgery (n=44), but two dogs that had undergone surgery with little tissue trauma (arthroscopy and orchiectomy) did not show any increase in CRP concentration 24 hours after surgery.

Of the dogs that were sampled 24 and 48 hours after surgery, five dogs had the highest CRP concentration 24 hours after surgery and five dogs had the highest CRP concentration 48 hours after surgery. Further studies of how long time after surgery to expect a peak in CRP concentration in the dog would be of value.

The serum concentrations of CRP 24 hours after surgery, for dogs that had CRP concentrations <30 mg/l preoperatively, ranged from 37 to 150 mg/l after osteotomy (n=9), from normal level (<5 mg/l) to 194 mg/l after arthroscopy or arthrotomy (n=8), from 12 to 69 mg/l after surgical stabilization of fracture/-s (n=4), from normal level (<5 mg/l) to 102 mg/l after orchiectomy (n=6), from 58 to 80 mg/l after prophylactic ovariohysterectomy (n=5) and from 38 to 165 mg/l after ovariohysterectomy due to uterus disorders (n=14). The wide range in CRP concentration in the different groups suggests that there are large individual differences in CRP response to surgery and that other factors than the type of surgical procedure have effects on the CRP level the day after surgery.

No correlation was observed between postoperative clinical signs and higher CRP levels compared to dogs without clinical signs that had undergone the same type of surgical procedure. But due to few dogs in the groups and varying types of postoperative clinical signs, no conclusions about CRP levels at postoperative complications could be made. Further studies of CRP and postoperative complications would be of value.
