Abstract

Tidigt i läkemedelsutvecklingen är det centralt att välja ut substanser som har potential att bli väl fungerande läkemedel. En faktor som tas stor hänsyn till är substansens in vitro potens. Det finns få studier kring hur in vitro potensen förhåller sig till terapeutisk plasmakoncentration av det färdiga läkemedlet hos patienten, men ett vanligt antagande är att den terapeutiska plasmakoncentrationen är en till tre gånger högre än in vitro potensen. I min litteraturstudie har jag fokuserat på följande frågeställningar:
- Hur förhåller sig in vitro potens till terapeutisk plasmakoncentration?
- Är det sätt som läkemedelsindustrin tolkar in vitro potens till terapeutisk plasmakoncentration optimalt?

In vitro potens-data och terapeutisk plasmakoncentration samlades in för nio etablerade läkemedelssubstanser. En kvot mellan terapeutisk plasmakoncentration och in vitro potens beräknades. Som modell för jämförelsen stod Göranssons (2012) examensarbete i farmaci som beräknade samma kvot för 150 etablerade läkemedel. Eftersom denna litteraturstudie endast omfattar några få substanser kan inte heller resultatet tolkas ensamt, utan endast i det sammanhang som Göranssons (2012) studie ger. Resultatet tyder på att de flesta läkemedel är mer potenta in vivo än in vitro. Detta innebär att en mer flexibel syn på in vitro potens tidigt i läkemedelsutvecklingen antagligen skulle kunna förhindra att bra substanser sållas bort i onödan.

An early step in the drug discovery process is to find out which compounds have the potential to become well-functioning drugs and therefore are suitable for further research. One factor that is considered is the in vitro potency of the compound. Few studies have described the relationship between in vitro potency and therapeutic plasma concentration. However, a common assumption is that the therapeutic plasmaconcentration is one to three times higher than the in vitro potency. The aim of this essay was to study this relationship by focusing on the following questions:
- How does therapeutic plasmaconcentration relate to in vitro potency?
- Is the way the pharmaceutical industry interprets in vitro potency optimal?
Therapeutic plasma concentration and in vitro potency data was collected for nine marketed drugs. A ratio between the two was calculated. A master´s thesis by Göransson (2012) made a similar comparison for 150 marketed drugs. Since nine substances are too few to draw any conclusions, it can only be correctly interpret in the context of Göransson’s study. The conclusion is that many drugs are more potent in vivo than in vitro, which means that drug discovery would probably benefit from a more flexible view on in vitro potency.
