Abstract

Cystisk endometriehyperplasi (CEH), är en mycket vanligt förekommande patologisk förändring hos framför allt äldre tikar, som innebär att körtelgångar i livmoder slemhinnan
fokalt dilateras och fylls med sekret så att cystor bildas. Dessa cystor kan bli uppemot en centimeter i diameter, ibland större, och anses uppkomma under diöstrusfasen av
löpningscykeln. Drabbade tikar är ofta symptomlösa men de kan uppvisa nedsatt allmäntillstånd, flytning och oförmåga att bli dräktiga. De har även en ökad risk att utveckla
allvarligare uteruspatologier såsom varbildande livmoderinflammation (pyometra).

I dennastudie har vi försökt bidra till kartläggningen av vad som händer i endometriet under utvecklingen av cystiska förändringar. Det finns olika sätt att histologiskt klassificera uterus patologier. Dow föreslog 1957 en
fyrgradig indelning, där grupp I innebär CEH, grupp II
innebär CEH med inflammatoriskt cellinfiltrat och grupp III & IV inkluderar olika typer av varbildande livmoderinflammation.

Trots att CEH är en vanlig patologisk förändring hos tik,
och trots att konsekvenserna kan bli allvarliga,
är patogenesen ännu inte helt utredd. Det är känt att tikar som inte har valpat är predisponerade, att det finns en genetisk komponent, samt att framför allt äldre tikar under
diöstrusfasen drabbas. Varför vissa körtelgångar omvandlas till cystor står dock obesvarat.

Syftet med detta arbete var att studera graden av vaskularisering i normalt endometrium jämfört med i cystiskt förändrat endometrium utan inflammation (Dow grupp I).

Hypotesen var att det finns fler kapillärer i normalt endometrium än i cystiskt förändrat endometrium. Patologiskt förändrad uterus vävnad från fem tikar samt normal uterusvävnad från fem tikar i diöstrus undersöktes
histologiskt avseende förekomst av olika stora kärl. Små kärl innebar att två eller färre endotelkärnor var synliga vid tvärsnittat kärl och stora kärl innebar tre eller fler
synliga kärnor.

I tvärsnittspreparat från dessa livmödrar räknades
kärlförekomsten i det endometriella stromat i representativt fördelade synfält motsvarande 20-34 % av den
endometriella snittytan i de tio preparaten.

Det fanns ett signifikant större antal små kärl i de normala preparaten jämfört med de cystiskt förändrade
(p=0,0367). Ingen signifikant skillnad förelåg i förekomsten av stora kärl hos de olika grupperna. De små kärlen återfanns framför allt i nära anslutning till körtelgångar, där kapillärer förekom i ansenlig mängd i preparaten från friskt endometrium. Runt de dilaterade
körtelgångarna i preparaten med cystiska förändringar var kapillärförekomsten synbart lägre.

En spekulation angående cystornas patogenes som kan göras utifrån detta resultat är att sämre blodförsörjning kring körtelgångar skulle kunna leda till att sekretet
inuti körtelgångarna får ökad viskositet, som leder till
fokala utvidgningar av körtelgångarna genom obstruktion.
Detta skulle överensstämma med att cystorna anses
uppkomma i diöstrus när sekretionen påtagligt minskar. Det skulle även vara förenligt med dräktighet som skyddande faktor då den medför ett bättre utvecklat kärlnät i livmodern vilket till viss del kvarstår också efter dräktigheten.

Cystic endometrial hyperplasia (CEH), is a very common pathological alteration in the uterus of the adult
bitch. In CEH, glandular tubules in the endometrium are
focally dilated and filled with glandular secretion
so that cysts are formed. These cysts have a diameter of up to one centimeter, occasionally larger, and are thought
to arise during the dioestral phase of the oestrous
cycle. Affected bitches are often a symptomatic but may present with a mildly reduced general condition, vulvular discharge and infertility. They also have an increased risk
of developing more severe lesions of the uterus such as pyometra. In the present survey, we have tried to contribute to a better understanding of the development of endometrial cysts.

There are various ways to classify uterine pathologies histologically. In 1957, Dow suggested a four -
point division, where group I means CEH, group II
consists of CEH with inflammatory cell ular infiltrates and
group III & IV in cludes different types of pyometra.
Although CEH is a frequently seen pathological alteration in the bitch, and although the consequences may be severe, the pathogenesis is not yet completely understood. It is known that nulliparous bitches are predisposed, that there are some genetic compon ents, and that especially elderly bitches are affected during the dioestral phase. However, why segments of the glandular ducts are converted into cysts
still remains unanswered.

The purpose of this study was to quantify the vascularization in normal endometrium compared with cystically altered endometrium without inflammation
(Dow group I).

The hypothesis was that there are more capillaries in the normal endometrium than in cystically altered endometrium.
Uterine tissue from five bitches with CEH and from five healthy bitches in dioestrus was examined histologically for presence of vessels of different size.

Small vessels meant that two or fewer endothelial cell nuclei were visible in cross section, while large
vessels had three or more visible nuclei. Evenly distributed areas corresponding to 20-34 % of the area were
analysed for vessel density in the ten transversely sectioned samples of the endometrium.

A significantly larger number of small vessels were found
in normal uterine tissue compared with cystically altered tissue (p = 0.0367). No significant difference was found between the large vessel density of the normal and the CEH tissue samples. The small vessels were found
particularly in close proximity to glandular ducts, especially in the samples from healthy endometrium. Around the dilated glandular ducts in samples with cystic alterations, the vessel density was visibly lower.

One speculation regarding the pathogenesis of the cysts that can be made from these results is that lower blood
supply around the glands could lead to increased viscosity of the fluid inside the glandular ducts and thus be the cause of the focal enlargements of these, by obstruction.
This would be consistent with the assumption
that the cysts are considered to arise in dioestrus when the glandular secretory activity of the bitch apparently decreases. It would also be consistent with pregnancy being
a protective factor, giving rise to a more developed uterine
vascularisation that to some extent remains also after the
pregnancy.
