Abstract
Mild-måttlig ekvin astma (MEA) är en vanlig orsak till nedsatt prestation hos hästar. Då lindriga förändringar redan tidigt märks av som försämrad prestation finns ett särskilt intresse för MEA inom trav- och galoppsporten. Fortfarande är olika MEA-fenotypers koppling till prestation och specifika potentiella agens av icke-infektiös och infektiös art inte helt förstådd. Infektion med
hästens spolmask Parascaris spp. har i enstaka studier föreslagits men inte säkert kunnat beläggas som etiologi till eosinofil MEA. Syftet med detta arbete var att undersöka förekomsten av MEA hos unga travhästar med upplevd nedsatt prestation vilket resulterade i ett studieupplägg i tre delar.
Arbetets första del var en fallkontrollstudie med syftet att jämföra två grupper á elva varmblodiga travare med och utan prestationsnedsättning. En klinisk undersökning, endoskopi av trakea och svalg och provtagning med bronkoalveolär lavage (BAL) utfördes. Inget samband sågs mellan
av tränarna uppfattad prestationsnivå och grad av luftvägsinflammation baserat på endoskopifynd.
Hästarna delades in i olika MEA-fenotyper baserat på de två idag accepterade gränsvärdena – ett högre för klinisk signifikans (neutrofiler >10 %, mastceller >5 %, eosinofiler >5 %) och ett lägre som utgör gränsen för friska hästar (neutrofiler >5 %, mastceller >2 %, eosinofiler >1 %). Förekomsten av MEA var oförväntat hög (18 respektive 66 %) i gruppen normalpresterande hästar.
Den låga förekomsten av MEA i den prestationsnedsatta gruppen (9 respektive 18 %) kan bero på att andra tillstånd var vanligare orsaker till nedsatt prestation i mål- och/eller studiepopulationen men att dessa inte kunde fastställas inom ramen för denna studie. Den höga förekomsten i den normalpresterande gruppen antas kunna bero på ett icke-representativt urval till studiepopulationen då tränarnas subjektiva uppfattning av hästarnas prestation var avgörande för tilldelning i fall- eller kontrollgruppen.
Den andra delen bestod av en retrospektiv genomgång av 279 provsvar från 233 varmblodiga travare mellan ett och fyra år som genomgått BAL-provtagning på en av två svenska hästkliniker sedan 2014. De klassificerades efter konsensusutlåtandets högre respektive lägre gränsvärdena för
att undersöka förekomsten av MEA-fenotyperna. Förekomsten av MEA var högre hos tvååringar (54 och 87 %) än hos tre- (31 och 75 %) och fyraåringar (31 och 75 %). Den vanligaste MEAfenotypen i den totala populationen var den eosinofila med 17 och 23 %, följt av den mastocytära (9 och 13 %), den neutrofila (7 och 6 %) och de blandade fenotyperna (totalt 4 respektive 35 %).
Särskilt hög var förekomsten av eosinofil MEA hos tvååringarna. Inga slutsatser kan eller bör dock dras om kopplingen till prestationsnedsättning baserat på denna studie då sökkriterierna inte innefattade orsaker till provtagningen.
Den tredje delen bestod dels av en enkätstudie där hållningsrelaterade faktorers roll i utvecklingen av förhöjda eosinofiler i BAL undersöktes. Utevistelse, grovfodertyp, utfodringsmetod, strömedel och förekomst av spolmaskinfektion hos en grupp med >1 % eosinofiler i BAL och en med normal BAL-cytologi jämfördes. I enlighet med tidigare studier sågs signifikanta skillnader i
användning av grovfodertyp (p <0,001) där 87 % av hästarna i gruppen med förhöjda eosinofiler fick hö, medan 83 % i gruppen med normal BAL-cytologi fick hösilage. Användningen av hö tros vara en orsak till eosinofil inflammation hos hästarna i denna studie. Även ätpositionen skiljde sig
åt signifikant (p <0,05) mellan grupperna, detta anses dock inte övertygande med tanke på ursprungsdatans heterogenitet. I övrigt sågs inga betydande skillnader mellan grupperna. Inget tyder på att migrerande spolmasklarver var orsaken till eosinofil astma i studiepopulationen. Baserat på
tidigare studier bör Parascaris spp. dock inte helt avsägas sin potential som orsak till eosinofil MEA.
Mild-moderate equine asthma (MEA) is a common cause of poor performance in horses. As even mild changes in pulmonary function may have a negative effect on their performance, MEA has been of particular interest in Standardbred and Thoroughbred racehorses. The association between
different MEA phenotypes with performance and specific potential non-infectious and infectious agents is yet to be fully understood. Parascaris spp. has been proposed as a potential cause of MEA. The aim of this study was to investigate the prevalence of MEA in young racehorses with
perceived poor performance resulting in a study design consisting of three separate parts.
The first part consisted of a case-control study of two groups where normally and poorly performing Swedish Standardbred trotters were compared. All horses were clinically examined and underwent endoscopy of the trachea and pharynx and bronchoalveolar lavage (BAL). No
association between performance levels reported by the horses´ trainers and endoscopy scores were found. Based on their BAL results each horse was assigned to a MEA phenotype using two different cut-off values from the latest consensus statement on the topic – either reaching clinical
significance (neutrophils >10%, mast cells >5%, eosinophils >5%) or exceeding the upper limit for healthy horses (neutrophils >5%, mast cells >2%, eosinophils >1%). The prevalence of MEA was unexpectedly high (18 or 66%) in the normally performing group which could be due to the
study population being a non-representative sample from the target population as allocation of the horses into the case or control group was based on the trainer’s perception of the horses’ performance. The low prevalence of MEA in the group with poor performance might be caused by a
higher prevalence of other conditions with a negative impact on performance in this population. In future studies a larger study population and a modified sampling process are recommended.
The second part of the study was a retrospective review of 279 BAL results from 233 individual Standardbreds between the ages of one and four years that had undergone BAL sampling at one of two Swedish equine clinics since 2014. These were divided into different phenotypes based
on the two cut-off values to determine the prevalence of the MEA phenotypes in both cases. Irrespective of the cut-off values, a higher prevalence of MEA was observed in two-year-olds (54 or 87%) than in three- (31 or 75%) and four-year-olds (31 or 75%). The prevalence of the different
MEA phenotypes in the total study population with the two cut-off values was as follows: eosinophilic 17 or 23%, mastocytic 9 or 13%, neutrophilic 7 or 6%, with the mixed inflammation types totalling 34 or 35%. No conclusions can or should be drawn regarding the association with poor
performance in the study population, as the search criteria did not include the reason for the performance of BAL.
The third part of the study addressed possible etiologies of increased eosinophils in the bronchoalveolar lavage fluid (BALF) by comparing the trainers´ questionnaire answers about management factors and the prevalence of Parascaris infection according to faecal samples in two
groups of trotters: one with >1% eosinophils in BALF, and one with normal BAL cytology. In accordance with previous studies, there were significant differences between the groups regarding the type of roughage used (p<0,001), as 87% of the horses with elevated eosinophils were fed hay,
while 83% of horses with normal BAL cytology were fed haylage. The feeding height also differed significantly between groups (p<0,05), which contradicts the results of previous studies and given the heterogeneity of the original data does not seem plausible. Migrating Parascaris
spp. larvae were not identified as the cause of eosinophilic asthma in this study. However, considering results from other previous studies, Parascaris spp. should not be completely dismissed as a potential cause of MEA based solely on the results of this study.
