Abstract

Leptospiros är bakteriell zoonos som under de senaste åren uppmärksammats i Sverige efter att ha orsakat akut sjukdom och dödsfall hos ett ökat antal hundar. Att diagnosticera sjukdomen är ut-manande, då det saknas ett enkelt, tillförlitligt test.

Syftet med studien var att undersökta hur fall av leptospiros hos hund diagnosticerats och hanterats på svenska veterinärkliniker och djursjukhus. Studien genomfördes som en retrospektiv journal-genomgång av 32 hundar med leptospiros. För varje fall inhämtades relevanta uppgifter som sam-manställdes och analyserades i Excel. Målet var att belysa den komplexa kliniska symptombild som drabbade hundar kan uppvisa samt de utmaningar som finns vid diagnostik och tolkning av provresultat.

Samtliga hundar uppvisade kliniska symptom och blodprovsavvikelser förenliga med leptospiros. Majoriteten av hundarna var yngre än ett år, och flest fall inträffade mellan september och decem-ber. De flesta hundarna vårdades i Skåne. PCR som diagnostik användes i fler fall än serologi, men i majoriteten av fallen kombinerades metoderna och samtliga provresultat tolkades tillsam-mans med den kliniska bilden. De vanligaste serovererna som identifierades var L. Icterohaemorr-hagiae och L. Sejroe. I samtliga fall dokumenterades vidtagna smittskyddsåtgärder på kliniker och djursjukhus. Behandlingen bestod i regel av antibiotika i kombination med understödjande vård.

Studien visar att diagnostiken av leptospiros hos hund är komplex och svår att standardisera, och att en tillförlitlig diagnos kräver både klinisk bedömning och god kännedom om de olika metoder-nas begränsningar. Den understryker även behovet av fortsatt nationell övervakning samt ett nära samarbete mellan myndigheter och veterinärer för att möjliggöra tidig identifiering och effektivt förebyggande av sjukdomen.

Leptospirosis is a bacterial zoonosis that has gained increased attention in Sweden in recent years due to a rising number of acute cases and fatalities among dogs. The diagnosis is challenging, as no single reliable test is available.

The aim of this study was to investigate how cases of canine leptospirosis have been diagnosed and managed at veterinary clinics and animal hospitals in Sweden. A retrospective review of medical records from 32 dogs diagnosed with leptospirosis was performed. Relevant clinical information and test results were compiled and analysed to highlight both the complex clinical presentation of affected dogs and the diagnostic challenges encountered.

All dogs showed clinical signs and laboratory abnormalities consistent with leptospirosis. The majority were younger than one year, and most cases occurred between September and December. Most dogs were treated in the region of Skåne. PCR was used more frequently than serology for diagnosis, although the methods were commonly combined and all results were interpreted alongside the clinical presentation. The most frequently identified serovars were Leptospira Icterohaemorrhagiae and L. Sejroe. Infection control measures were documented for all cases in clinics and animal hospitals, and treatment typically consisted of antibiotics alongside supportive care.

The result indicate that diagnosing leptospirosis in dogs is complex and difficult to standardize, and that a reliable diagnosis requires an integrated assessment of clinical findings and laboratory results together with an understanding of each method’s limitations. The study also underscores the need for continued national surveillance and close collaboration between veterinarians and public health authorities to enable early detection and effective prevention of the disease.
