Abstract

Cystatin C är ett protein och en proteinasinhibitor med låg molekylärvikt (13kDa) som är involverat i intracellulär proteinkatabolism och produceras med konstant hastighet. Urin-cystatin C (uCysC) hos en frisk hund resorberas fullständigt i proximala tubuli och finns normalt i mycket små, ej mätbara mängder i urinen. Ökad koncentration av uCysC har setts hos hundar med experimentella toxiska njurtubuliskador.

Syftet med studien var att undersöka om cystatin C i urin kan vara en presumtiv markör för njurskada hos hundar med olika typer av allvarliga sjukdomstillstånd. Urinprover samlades in från 46 hundar som uppsökte vård på SLU Universitetsdjursjukhus akutmottagning, hundarna hade nedsatt AT och var i behov av vätskebehandling. Urinproverna analyserades för uCysC, urinprotein och u-kreatinin. Data från journaler och urinproverna sammanställdes och statistisk analys genomfördes.

Hos de 46 provtagna hundarna hade 11 (24 %) förhöjt cystatin C i urinen. Av de 11 hundarna med förhöjt uCysC (grupp A) hade fem hundar olika former av akutisering av kronisk njursjukdom (ACKD), och sex hundar hade systemisk inflammatorisk sjukdom eller var ormbitna (Vipera berus). De resterande 35 hundarna hade uCysC nivåer under mätområdet (grupp B). Urinprotein-/ukreatinin-kvot (UPC) och serum-kreatinin (sCr) var signifikant förhöjt i grupp A jämfört med grupp B. Hundarna i grupp A var signifikant mer cirkulatoriskt påverkade, samt hade signifikant mer påverkat allmäntillstånd jämfört med grupp B.

I studien var det ett antal hundar med systemisk inflammation eller ormbett som hade förhöjt uCysC i kombination med förhöjd urinprotein/u-kreatinin-kvot, vilket kan tala för sekundär njurskada hos dessa patienter. Ökning av cystatin C i urin kunde även observeras hos hundar med akutisering av kronisk njursjukdom. Cystatin C i urin har potential som markör för akut njurskada på hund men mer forskning med större studiepopulationer behövs för att kunna dra slutsatser om den kliniska användbarheten för denna urinmarkör och om den möjliggör tidigare upptäckt av akut njurskada hos hundar.

Cystatin C is a protein and a proteinase inhibitor with low molecular weight (13 kDa) that is involved in intracellular protein catabolism and is produced at a constant rate in all cells in the body. Concentrations of cystatin C (uCysC) in a healthy dog is reabsorbed in the proximal renal tubuli and exist in very small/unmeasurable amounts in urine. An increase in the concentration of uCysC has been shown in dogs with experimental toxic kidney tubuli damage.

The objective of this study was to investigate if cystatin C can be a presumptive marker for kidney injury in dogs with different types of serious medical conditions. Urine samples was collected from 46 dogs that were seen in the emergency room at SLU University Animal Hospital. The urine was analyzed for uCysC, u-protein and u-creatinine. Data from the dog’s medical records and the urine analysis where compiled and statistical analysis was performed.

Eleven dogs (24%) had elevated uCysC concentrations when urine samples from 46 dogs where analyzed. Five out of the 11 dogs with elevated uCysC (group A) concentrations were diagnosed with acute on chronic kidney disease (ACKD), and six dogs had systemic inflammation or snake bite toxicity (Vipera berus). The remaining 35 dogs had concentrations of uCysC below measurable concentrations (group B). The urine protein-/u-creatinine-ratio (UPC) and serum-creatinine (sCr) was significantly elevated in group A when compared to group B. Group A had significantly more affected general condition and was significantly more circulatory affected compared to group B.

There were some dogs in the study with systemic inflammation or snake bite toxicity that had increased concentrations uCysC in combination with an increased urine protein-/u-creatinine-ratio, this can indicate secondary kidney injury within these group of dogs. Urin-cystatin C was also elevated in dogs with acute on chronic kidney disease. Cystatin C is showing potential as a marker for acute kidney damage in dogs, but more research needs to be done on a larger study population to be able to draw conclusions about the clinical relevance of the marker in urine and if it makes it possible to detect kidney injury at an earlier stage in dogs.
