Abstract

Arteriell syremättnad i blodet är en livsviktig parameter att monitorera vid anestesi och intensivvård
inom djursjukvård. Otillräcklig syresättning kan leda till hypoxi, syrebrist i vävnaden, och åtgärdas
det inte kan det i värsta fall medföra att patienten avlider. Arteriell syremättnad kan mätas
kontinuerligt med pulsoximetri eller vid en specifik tidpunkt genom arteriell blodgasanalys. Inom
pulsoximetri finns två olika mätningstekniker; transmission- och reflektansteknik. Med
transmissionteknik är lysdioderna och fotodioden placerade på motsatt sida av vävnaden medan de
i reflektansteknik är placerade på samma sida av vävnaden. Reflektansteknik används endast i liten
utsträckning inom djursjukvård och relativt få artiklar inom djursjukvård beskriver reflektansteknik
till skillnad från humansjukvård där reflektansteknik omnämns i flertalet artiklar.
I denna studie undersöktes huruvida det var möjligt att mäta arteriell syremättnad och hjärtfrekvens
med reflektansteknik på djur och vilka placeringarna som var de mest optimala för proben. Studien
genomfördes under nio dagar på Universitetsdjursjukhusets operationsavdelning och totalt 21 djur
ingick i studien, varav 17 hundar och fyra katter. En pulsoximeterprob med reflektansteknik
placerades på olika områden på kroppen och uppmätt syremättnad och hjärtfrekvens noterades. På
en del av patienterna var även en pulsoximeterprob med transmissionteknik placerad. Resultaten
som erhölls från respektive teknik jämfördes. Ingen arteriell blodgas analyserades under studien.
Med jämna mellanrum palperades femoralartären och hjärtfrekvensen noterades. Denna jämfördes
med hjärtfrekvens uppmätt med respektive teknik. Resultatet påvisade ett starkt positivt samband
mellan syremättnad och hjärtfrekvens uppmätt med transmission- och reflektansteknik. Det visade
även ett starkt positivt samband mellan palperad hjärtfrekvens och hjärtfrekvens uppmätt med
reflektansteknik. Flest resultat med reflektansteknik gavs från svansrot och ljumske, dock placerades
även reflektansproben flest antal gånger på dessa områden. Därmed kan ingen slutsats dras om vilka
områden som är de mest optimala för mätning med reflektansteknik.

Arterial oxygen saturation in the blood is a vital parameter to monitor, especially during anesthesia
and intensive care in the veterinary field. Insufficient oxygen saturation can cause hypoxia, low
oxygen levels in the tissue, and if it is not addressed it can in worse case lead to death. Arterial
oxygen saturation can be monitored continuously with pulse oximetry, or at a specific time with
arterial blood gas analysis. There are two pulse oximetry measuring techniques; transmission- and
reflectance technique. Transmission technique has the LED lights and the photodiode placed on the
opposite side of the tissue, while reflectance technique has them placed on the same side of the
tissue. Reflectance technique is not commonly used in veterinary care. There are just a few articles
about reflectance technique from the veterinary field compared to the human health field.
This study examined whether it was possible to measure arterial oxygen saturation and heart rate in
dogs and cats with the help of reflectance technique, and which areas on the body that were the most
optimal to place the probe. The study was conducted over nine days in the surgery room at the
University animal hospital. A total of 21 pets participated, 17 dogs and four cats. A pulse oximetry
probe with reflectance technique were placed at different measuring areas of the body, and measured
oxygen saturation and heart rate were noted. Some of the patients also had a transmission probe
placed. The results obtained from each technique were compared. No arterial blood gases were
analysed in this study. The heart rate was periodically palpated from the femoral artery. It was
compared to the heart rate measured with each technique. The result showed a strong positive
correlation between oxygen saturation and heart rate measured with transmission and reflectance
technique. It also showed a strong positive correlation between palpated heart rate and heart rate
measured with reflectance technique. Most results were recieved from the base of the tail and the
groin, however, the probe was placed the most number of times in these measurement areas.
Therefore, no conclusion can be drawn as to which areas are the most optimal for measurement with
reflectance technique.
