Abstract

Inflammation i ögats uvea, så kallad uveit, är en smärtsam och vanlig komplikation till olika
sjukdomstillstånd i hästars ögon. Vid uveit uppstår en spasm i pupillen och i ciliarkroppen vilket är
smärtsamt och komplikationer i form av sammanväxningar (så kallade synechier) mellan lins och
iris kan leda till kvarstående synnedsättning eller blindhet. Atropin är ett antikolinergikum som
relaxerar iris och ciliarmuskulaturen vilket minskar smärtan samt risken för synechier. Atropin är
en antagonist på muskarinreceptorer som förutom i ögat återfinns bland annat i mag-tarmsystemet.
Därför kan atropin ge nedsatt tarmmotilitet och kolik hos hästar både vid topikal och intravenös
administrering beroende på vilken dos som administreras.
På grund av att atropin kan ge kolik har medfört en rädsla inför hur läkemedlet bör användas och
sannolikt har en alltför restriktiv behandlingsnorm uppstått, där vissa veterinärer helt undviker
atropin vid behandling av uveit (Ström, L., resident i oftalmologi vid Sveriges lantbruksuniversitet,
pers. medd., 20191102). Syftet med detta examensarbete är att farmakologiskt utvärdera atropins
förlopp med PK/PD-modellering och utvärdera metoderna elektrointestinografi (EIG) samt
auskultation av tarmljud för att hitta gränsvärden för vilken plasmakoncentration av atropin i blodet
som ger nedsatt myoelektrisk aktivitet i tarmen.
Detta examensarbete bygger på en litteraturdel, som motiverar de metoder som använts, och en
experimentell del. I den experimentella delen har 8 hästar deltagit i två omgångar där de ena
omgången erhållit en konstantinfusion med atropinsulfat i 30 minuter och andra omgången
natriumkloridlösning (kontroll) i en blindad och randomiserad cross-over design. Fyra av hästarna
erhöll dosen atropinsulfat 9 µg/kg och fyra hästar 12 µg/kg. Observera att 1 µg atropinsulfat
motsvarar 0,835 µg atropin. Under och efter infusionen mättes tarmens elektriska potential med
EIG under tio timmar genom elektroder placerade på huden i området för caecum och colon.
Dessutom auskulterades tarmljuden med stetoskop och plasmakoncentrationerna med atropin
mättes under tio timmar vid på förhand fastställda tidpunkter. Koliksymptom noterades och
graderades från mild – svår utifrån särskilda kriterier. Farmakologiska data analyserades och
användes för farmakologiska modeller i mjukvaran Monolix©Lixoft.
Samtliga hästar fick signifikant (p < 0,000 – 0,048) nedsatta EIG-potentialer under försökets första
20 minuter – 3 timmar efter att infusionen med atropinsulfat startats. Detta korrelerade med
tarmljuden som blev signifikant (p < 0,000 – 0,012) nedsatta mellan försökets första 15 minuter och
2 timmar efter att infusionen startats. Sex hästar uppvisade milda koliksymptom (grad 1/6) någon
gång under försökets första 29 – 75 minuter efter att de erhållit atropinsulfat, koliksymptom
noterades även hos en häst efter att den erhållit natriumkloridlösning. Efter modellering i mjukvaran
Monolix©Lixoft uppskattades att maximala hämningen (Imax) av EIG-potentialen i caecum var 55,8
% och halva hämningen (IC50) uppnåddes vid plasmakoncentrationen 0,343 µg/L. I colon var Imax
66,7 % och IC50 0,59 µg/L.
Dessa experiment bekräftar att resultaten korrelerar väl vid mätning av EIG jämfört med när
tarmljuden observeras med stetoskop. Dessutom visade hästarna endast milda koliksymptom, trots
att de fick höga doser atropin jämfört med de doser som används kliniskt. Hur olika koncentrationer
atropin påverkar tarmens elektiska potential kartlades och den doseringsregim med atropin som
används i dagsläget vid behandling av uveit bedöms inte kunna ge upphov till kolik.

Inflammation in the uvea, so called uveitis, is a painful and common complication to various
pathological events in the horse eye. Uveitis causes a spasm in the pupil and ciliary body of the eye
which is painful and complications as adhesions (so called synechiae) between the lens and the iris
can cause permanent loss of vision or blindness. Atropine is an anticholinergic drug that relax the
iris and ciliare body which reduce pain and the risk for synechiae. Atropine is an antagonist on
muscarinic receptors which can be found in the eye and in the gastrointestinal tract. Hence atropine
can reduce gastro intestinal motility and cause colic in horses after either topical or intravenous
administration, depending on the dose.
The fact that atropine can cause colic has led to uncertainty if the drug should be used and a
treatment standard that is probably too restricted has arisen, where some veterinarians exclude
atropine when treating uveitis (Ström, L., resident in ophthalmology at Swedish University of
Agricultural Sciences, pers. medd., 20191102). The aim of this thesis is to evaluate the
pharmacology of atropine in the body of the horse with PK/PD-modelling, evaluate the methods
electrointestinography (EIG) and auscultation of intestinal sounds in order to find limits of plasma
concentration of atropine that lower the myoelectric activity in the intestine.
This thesis is based on a literature study that motivates the methods used and an on experiments. In
the experiments 8 horses participated in two sets. In one set the horses received a constant infusion
with atropine sulfate for 30 minutes and in the other set sodium chloride (control group) in a blinded
randomized cross-over design. Four of the horses received atropine sulphate in a dose of 9 µg/kg
and four of the horses received 12 µg/kg. Note that 1 µg atropine sulphate is equal to 0.835 µg
atropine. During and after the infusion the gastrointestinal myoelectrical potentials were measured
with the EIG method for ten hours using electrodes placed on the skin in the area of caecum and
colon. The gastrointestinal sounds were auscultated with a stethoscope and the plasma
concentration of atropine was measured during ten hours at specific times that were planned in
berforehand. Symptoms of colic were noted and graded from mild to severe based on particular
criteria. The pharmacological data were analyzed and used in pharmacological modelling in the
software Monolix©Lixoft.
All horses had a significant (p < 0,000 – 0,048) decrease of EIG potentials during the first 20
minutes to 3 hours after the start of the atropine sulphate infusion. This was correlating well with
the intestinal sounds that were significantly (p < 0,000 – 0,012) decreased during the first 15 minutes
to 2 hours after start of infusion. Six horses showed mild signs of colic (grade 1/6) some occasion
during the first 29 – 75 minutes after the infusion started. Mild signs of colic were noted in one
horse after receiving sodium chloride infusion. After establishing a pharmacological model in the
software Monolix©Lixoft the maximal inhibition (Imax) of the EIG potential in caecum was 55,8%
and half of the inhibition effect (IC50) was achieved at the plasma concentration 0,343 µg/L. In
colon the Imax was 66,7% and the IC50 was 0,59 µg/L.
These experiments confirm that the results correlate when using EIG compared to auscultation
intestinal sounds with stethoscope. The horses only showed mild signs of colic although the
received relatively high doses of atropine sulphate. The response of the myoelectrical potentials in
the intestines due to different plasma concentrations of atropine were plotted and the current dosage
of atropine when treating uveitis does not seem to be a possible cause of colic in horses.
