Abstract

Alla betande hästdjur infekteras med endoparasiter. Behandling med avmaskningsmedel är en
del i att kontrollera smittspridning och förhindra sjukdom av parasitär etiologi. Cyathostominae
spp. (små blodmaskar) har ägnats mycket uppmärksamhet på senare år eftersom att prevalensen
praktiskt taget är 100 % och många populationer av lilla blodmasken är resistenta mot två av
tre verksamma anthelmintikasubstanser. Rapporter om resistens mot den sistnämnda substansen, makrocykliska laktoner (ML), finns från flera håll världen över, och nya behandlingsprogram har utformats i syfte att bevara ML:s effekt så länge som möjligt. På Island har
avmaskning av hästar med bland annat ivermektin pågått i flera decennier. Den inhemska
hästpopulationen har varit isolerad i 1100 år, vilket medför att även dess parasitpopulationer
har varit det. Därför är eventuell resistens hos islandshästars parasiter intressant att studera då
denna har utvecklats oberoende av genutbyte med parasitopulationer från övriga världen. Syftet
med detta examensarbete var att undersöka ivermektins effekt på lilla blodmasken hos naturligt
infekterade föl på Island genom Modifierat McMaster-test och faecal egg count reduction test
(FECRT). Tio gårdar medverkade i studien vilket resulterade i 85 provtagna föl varav 10
tillhörde en kontrollgrupp. Parprover togs i samband med att fölen avmaskades med subkutan
injektion med ivermektin, respektive två veckor senare. En godtagbar behandlingseffekt av
blodmask efter ivermektin har definierats som en minskning av äggurskiljningen med ≥ 95 %,
då visar testen att resistens inte föreligger. Fyra av gårdarna exkluderades från studien eftersom
att de inte uppfyllde inklusionskriteriet att minst fyra föl skulle urskilja > 100 epg vid första
provtagningstillfället, detta resulterade i att sex gårdar inkluderades till analys av FECRT.
Resultatet blev att samtliga behandlade föl hade minskad äggurskiljning vid den andra provtagningen. Detta gäller inte kontrollgruppen, där sex av tio föl fick ökade epg-nivåer mellan
provtagningarna och därav blev FECR endast 4,4 %. FECR blev 100 % hos två av gårdarna
och 99,6 % hos en tredje gård, vilka alla innebär en god effekt. En gård hade FECR på 95,7 %,
vilket ligger inom intervallet där resistens kan misstänkas (<98 %). De övriga två gårdarna låg
under 95 % reducering, på 92,3 % respektive 87,6 %, vilka motsvarar kriterierna för en resistent
parasitpopulation. Sammanfattningsvis visar resultatet att effekten överlag var hög och att det
finns känsliga likväl resistenta parasiter hos det befintliga urvalet. Innan slutsatser dras måste
andra orsaker till bristande behandlingseffekt tas i åtanke. Utöver detta var antalet provtagna
individer få och slumpmässigt urval tillämpades ej, varför slutsatser om resistensläget mot
ivermektin hos populationer av Cyathostominae spp. på Island i stort inte kunde dras. På 1990-
talet gjordes en studie där förkortat egg reappearance period (ERP) uppmättes på isländska
blodmaskar efter behandling med injektion med ivermektin. Det finns även en studie som visar
att effekten av ivermektin är ofullständig vid parenteral administrering jämfört med oral
tillförsel av ivermektin. Resultatet från studien tyder på att populationens tolerans har ökat
sedan 1990-talet. Rekommendationen till hästägarna blir att konfirmera effekten av vald
avmaskning med FECRT och i de fall där den är bristfällig avråds parenteral administration av
ivermektin för att förhindra fortsatt selektion för resistens. Förutom att optimera behandlingsregimer för den isländska hästnäringen, kan fortsatt forskning på resistensläget på Island öka
förståelsen för förvärvandet av resistensgener hos parasiter.

All grazing horses are infected with endoparasites. Treatment with anthelmintic drugs is a part
of prevention of infection and illness of parasitical etiology. In recent years, Cyathostominae
spp. (small strongylids) have received attention, since the prevalence is almost 100% and
resistance to two out of three anthelmintic drug classes has developed among many parasite
populations. Reports of resistance against the last drug class, macrocyclic lactones, are also
increasing and for that reason, new guidelines for deworming programmes have been developed
in order to maintain the efficacy of the existing substances. On Iceland, deworming of horses
has consisted of subcutaneous injection of ivermectin for several decades. The domestic
population of horses have been isolated for approximately 1100 years, which also include the
isolation of their parasites. Hence, eventual resistance among Icelandic horse’s parasites has
developed independently from genetic exchange with other parasite populations.
The aim of this thesis was to investigate the effect on the small bloodworm in naturally infected
Icelandic foals treated with subcutaneous injection with ivermectin, using the McMaster Test
and faecal egg count reduction test (FERCT). Ten farms with 85 foals including 10 foals
belonging to a control group participated in the study. Paired samples were collected at the
event of deworming and two weeks later, respectively. An acceptable drug efficacy after
treatment of bloodworms with ivermectin is 95%. Because of the inclusion criteria four farms
were excluded since they had less than four foals that excreted >100 epg on the first sampling
occasion. This resulted in six farms to be included in the FECRT.
All foals had a decreased egg excretion at the second sampling. This did not apply to the control
group were the FECR was only 4,4% due to an increase of epg excretion in six out of ten foals.
Two farms had FECR of 100% and one farm 99,6%. Another farm had a reduction of 95,7%
which entails that resistance could be suspected (<98%). The last two farms had less than 95%
reduction and thus met the criteria for resistance at 92,3% and 87,6%, respectively. In summary,
the results show a high efficacy of ivermectin, but nonetheless criteria for resistance were met
at two farms. Before conclusions can be drawn, other reasons for lack of therapeutic effect must
be taken into consideration. Furthermore, the number of sampled foals was low and the
selection of participating farms was not randomized. Therefore, conclusions of the status of
resistance in Cyathostominae spp. on Iceland could not be made. In the 1990’s a study was
conducted on Iceland were a shortened egg reappearance period (ERP) for treatment with
ivermectin as injection was found. Another study found that the efficacy of parenterally
administrated ivermectin in cyathostomins was lower compared to orally administrated
ivermectine. The result of the study indicates that the tolerance in the population has increased
since the 1990’s. Therefore, the recommendation to horse owners is to confirm the efficacy of
the chosen anthelmintic treatment with FECRT and if the result is insufficient, parenteral
administration is dissuaded to prevent further selection of resistance genes. Beyond improving
the treatment regimens for the Icelandic horse community, further research can improve the
understanding of the mechanisms of anthelmintic resistance in a bigger context and hopefully
find ways to prevent its development.
