Abstract

Det finns idag ett antal olika metoder för assisterat uppvak efter allmän anestesi av häst. Behovet att
utveckla och undersöka nya metoder är stort, då uppvaket anses vara en kritisk period där många
skador och dödsfall inträffar. Frakturer och myopatier står för en tredjedel av de fatala, postoperativa
komplikationerna. För- och nackdelar med olika typer av assisterade uppvak har observerats i
flertalet studier. Metodens effekt, säkerheten för hästar och personal samt ekonomisk hållbarhet är
faktorer som spelar stor roll för att ett assisterat uppvak ska anses fungera och ha önskad effekt i en
klinisk verksamhet.
Syftet med den aktuella studien var att undersöka om användning av justerbart tak i
uppvakningsboxen påverkade ponnyerna på ett sådant sätt att uppvakningskvaliteten förbättrades
och skaderisken minskade. Pilotstudien utformades som en crossover-studie. Två försöksomgångar
genomfördes där sex shetlandsponnyer fick vakna med respektive utan nedsänkt justerbart tak i
uppvakningsboxen efter allmän anestesi utan kirurgiskt ingrepp. Taket hissades upp om ponnyn
reste sig trots att taket var nedsänkt alternativt om ingen resning gjorts efter 30 minuter från att
ponnyn placerades i uppvakningsboxen. Uppvaken filmades och bedömdes i efterhand, både
kvantitativt och kvalitativt.
Resultatet visade att det justerbara taket inte kunde hålla ponnyerna nere den tiden som
förväntades. Däremot förbättrades uppvakningskvaliteten signifikant och ett signifikant mindre
antal resningsförsök registrerades för försöksomgången med det justerbara taket. Detta är en
indikation på att metoden kan vara effektiv för att minska skaderisken i uppvakningsboxen efter
allmän anestesi hos häst. Vidare kliniska studier med ett större urval krävs för att kunna dra tydligare
slutsatser för en större hästpopulation.

There are at present several methods of assisted recovery used for horses that have undergone
general anaesthesia. The need for process improvement and research into new methods is pressing,
on account of the post-operative phase being so critical. A high rate of mortality and frequency of
injury have been reported. Furthermore, studies show that a third of the fatal outcomes during
recovery are due to fractures and myopathies. Advantages and disadvantages have been researched
for the methods in use today. The level of safety for the horses as well as personnel, how effective
the method is and whether it is economically sustainable are common factors for an assisted recovery
to work in a clinical setting.
The purpose of this study was to investigate if the use of an adjustable ceiling in the recovery
box could affect the ponies in such a manner that the quality of the recovery improves and the risk
of injury decreases. This pilot study was designed as a crossover. Six Shetland ponies underwent
two separate events of recovery with and without the adjustable ceiling in the recovery box. The
ponies were not subjected to any surgical stimuli. The ceiling was raised if the pony made a
successful attempt to stand with the ceiling lowered or if the time limit of 30 minutes within being
placed in the recovery box had passed. The ponies were recorded in the recovery box and the footage
was on a later occasion reviewed based on both quantitative and qualitative aspects.
Results show that the adjustable ceiling could not keep the ponies in a recumbent position during
the estimated optimal timeframe predicted. The recovery quality on the other hand significantly
improved and the number of attempts to stand was significantly reduced in the recoveries with the
adjustable ceiling. This is an indication that the method can be effective in reducing the risk of injury
in the recovery box for horses that have undergone general anaesthesia. Further clinical studies and
a larger sample size are required to make more grounded conclusions.
