Abstract

Perioperativ hypotermi är ett tillstånd som kan uppstå vid anestesi och som kan or-saka allvarliga komplikationer, framför allt under den postoperativa perioden. Att monitorera patientens temperatur är därför en viktig del av djursjukskötarens arbete. Hypotermi är relativt väl utforskat hos människor och smådjur och forskningen visar att perioperativ hypotermi förekommer ofta. Den generella uppfattningen bland djur-hälsopersonalen är att perioperativ hypotermi sällan förekommer hos vuxna hästar, då en stor kroppsmassa antas minska risken för kraftig värmeförlust. När det gäller vuxna hästar finns endast ett fåtal studier gjorda där perioperativ hypotermi under-söks. De studier som utförts tyder dock på att även vuxna hästar tappar i temperatur vid anestesi och därmed, likt övriga djurslag, riskerar perioperativ hypotermi. Något som bland annat kan ha negativ inverkan på uppvakningskvaliteten. Den post-operativa perioden hos häst är sedan tidigare känd för att vara förenad med allvarliga risker, vilka vanligen inte drabbar smådjur i samma utsträckning. Det kan handla om svårigheter att resa sig alternativt allvarliga skelett- och/eller muskelskador, vilka kan leda till kritiska konsekvenser för hästen.

Syftet med det här arbetet var att utforska förekomst av perioperativ hypotermi hos vuxna hästar och undersöka vad som redogörs om ämnet i vetenskaplig litteratur. För att besvara syftet och frågeställningarna utfördes en litteraturstudie samt en praktisk studie på två hästkliniker i Sverige. Litteraturstudien undersökte befintlig forskning inom området hos häst och omfattade även studier från människor och hundar för att få tillräckligt litteraturunderlag. I den praktiska studien inkluderades totalt åtta hästar, vilkas perioperativa temperaturer monitorerades under elektiva och icke-akuta ingrepp. Temperaturmätningarna gjordes med en rektaltermometer och utfördes med regelbundet intervall från före premedicinering, under anestesin och fram till hästen stod upp i uppvakningsboxen.

Såväl den praktiska studien som resultat från litteraturstudien visade att hypotermi ofta förekommer intra- och postoperativt hos vuxna hästar. Hypotermi definierades utifrån en kroppstemperatur under 37,0 ℃, vilket uppmättes hos 75 procent av de hästar som inkluderades i studien. Tidigare studier har påvisat en högre förekomst av hypotermi än vad resultaten i den här studien kunde konstatera.

Konklusionen av denna studie var att perioperativ hypotermi förekommer hos vuxna hästar och dessa resultat stöds av utförd litteraturstudie. Dessutom tyder veten-skaplig litteratur på en korrelation mellan intraoperativ hypotermi och en högre kom-plikationsrisk, framför allt postoperativt. Därmed kan eventuella förebyggande eller behandlande åtgärder för hypotermi vara av betydelse för uppvakningskvaliteten och återhämtningen efter anestesi hos vuxna hästar.

Perioperative hypothermia is a serious complication that may occur due to anesthesia. Monitoring the patient’s temperature is therefore an important part of the veterinary nurse´s work. Hypothermia is relatively well researched in humans and small animals. The research shows that perioperative hypothermia frequently occurs and may cause several serious complications for the patient, especially during the postoperative period. The general perception amongst animal health personnel is that perioperative hypothermia rarely occurs in adult horses due to their large body mass, which is believed to reduce the risk of severe heat loss. There are few studies on perioperative hypothermia in adult horses. However, existing studies show that horses lose body heat during anesthesia and thus, like other animal species, risk hypothermia.

Research has shown that intraoperative hypothermia has a negative impact on hor-ses’ recovery quality. It is well known that there are serious risks associated with the postoperative period for the equine patient, which usually do not affect small animals to the same extent. The risks include difficulties to get to standing position or severe damage to skeletal and/or muscle, which may lead to critical consequences for the horse.

The aim of this study was to explore the occurrence of hypothermia in horses during anesthesia. To do this, a literature study and a practical study were carried out on two horse clinics in Sweden. The literature study reviewed existing research in the field and also included studies of humans and dogs to obtain sufficient amount of literature. The practical study included a total of eight horses, whose perioperative temperature was monitored during elective and non-acute procedures. The tempera-ture measurements were made with a rectal thermometer and were performed re-gularly from before pre-medication, during anesthesia and up until the horse stood up in the recovery.

The results of both practical study and literature study showed that hypothermia often occurs in adult horses intra- and postoperatively. The study found hypothermia, which was defined as a body temperature below 37,0 ℃, in 75 percent of the horses. Previous studies have shown a higher incidence of hypothermia than the findings in the present study.

The conclusion of this study was that perioperative hypothermia occur in adult horses and these results are supported by the literature study. Further more, scientific literature indicates a correlation between intraoperative hypothermia and increased risk of complications, especially postoperative. Thus, any preventative or treating measures can be of importance for the horses recovery quality after anesthesia.
