Abstract

Kolik är ett smärtsamt tillstånd i bukorganen som är vanligt förekommande hos häst. Allvarlighetsgraden varierar, men kolik är en vanlig orsak till avlivning. Behandling av kolik sker antingen medicinskt eller kirurgiskt under allmän anestesi. Tiden i uppvakningsboxen efter allmän anestesi är för hästar förenad med stora risker. Fatala komplikationer under uppvakningen hos häst efter allmän anestesi inkluderar frakturer, dislokationer av leder, postoperativ myopati, neuropati och kardiovaskulär kollaps. Hästar som genomgått akut laparotomi på grund av kolikdiagnos har en högre mortalitet under uppvakningsperioden än friska hästar som genomgått elektiva ingrepp. Få studier har inkluderat akuta ingrepp vid undersökningar om vad som påverkar kvaliteten på uppvakningen och vad som kan vara orsaker till att komplikationer uppstår.

Syftet med studien var att undersöka om tiden en häst är under allmän anestesi vid en akut laparotomi, föranledd av en kolikdiagnos, påverkar kvaliteten på uppvakningen. I studien undersöktes även om tiden hästen tillbringade i olika positioner, från det att hästen placerades i uppvakningsboxen tills dess att den stod upp, påverkade uppvakningskvaliteten. Underlaget till studien insamlades under 13 månader vid Universitetsdjursjukhusets hästklinik, Uppsala, och omfattar 24 hästar som opererats akut på grund av kolik.

Resultatet från den här studien visade att en kortare anestesitid resulterade i en bättre kvalitet på uppvakningen. Ingen tidsparameter från uppvakningsboxen kunde i denna studie påvisa en inverkan på uppvakningskvaliteten. Ytterligare parametrar som i studien påverkade uppvakningens kvalitet var operationstid, hästens ålder och storlek på uppvakningsbox. Vidare studier av hästar som genomgått akut laparotomi krävs, men resultatet är en indikation på att kortare anestesitid kan leda till bättre kvalitet på uppvakningen.

Colic is a painful condition of the abdominal organs and commonly seen in horses. The severity varies but colic is a common reason for euthanasia. Treatment of colic is either medical or surgical. Fatal complications during the recovery time after general anaesthesia in horses include fractures, joint dislocations, postoperative myopathy, neuropathy and cardiovascular collapse. Horses that have undergone acute laparotomy due to colic have a higher mortality rate during the time in the recovery box than healthy horses that have undergone elective surgical procedures. Few previous studies have included acute procedures while researching what affects the quality of the recovery and what may be the cause of complications.

The purpose of the study was to investigate if the time a horse is under general anaesthesia during an emergency laparotomy, due to colic, affects the quality of the recovery. The study also investigated whether the time the horse spent in different positions, from when it was placed in the recovery box to the point that it was standing up, affected the quality of the recovery. Data was collected over a 13-month period at the University Animal Hospital’s horse clinic, Uppsala, and include a total of 24 patients that had undergone acute surgery due to colic.

The results of this study showed that a shorter time spent under general anaesthesia resulted in a better quality of recovery. No parameter of the time spent in the recovery box could, in this study, be found to affect the recovery quality. Additional parameters that were found to affect the recovery quality were; the horse’s age, duration of surgery and size of recovery box. Further studies of horses that have undergone acute laparotomy are required, but the results of this study indicate that a shorter time under general anaesthesia may lead to a better recovery quality.
