Abstract

En av de vanligare dödsorsakerna hos domesticerade katter är kronisk, progressiv njursjukdom, Chronic Kidney Disease (CKD). Vid CKD har njurarna skadats till den grad att irreversibla förändringar som nekrotiserar nefron uppstått och därigenom försämrar njurkapaciteten. Sjukdomen uppkommer vanligtvis bland äldre individer. Kliniska fynd inkluderar bland annat proteinuri, hyperkalemi, hypokalcemi, viktnedgång och palperbart små njurar.

Vid diagnos CKD graderas vanligen sjukdomens progression hos patienten. Detta görs genom International Renal Interest Societys skala, som är baserad på grad av kreatininhalt i blodet, systoliskt blodtryck samt proteinhalt i urinen. Höga värden på parametrarna, framför allt halten kreatinin, tyder på långt framskriden sjukdom och utefter det anpassas eventuell uppföljning och behandling.

I dagsläget är etiologierna för CKD inte fullständigt utredda men det finns vissa teorier. Ett antal av dessa teorier är dokumenterade, varav några redogörs för i denna studie. Genom till exempel blodprover, mikroskopisk bild och anamnes kan man få en idé om hur sjukdomen uppkommer och utvecklas hos individen. Mikroskopiska fynd som hittas i njuren hos alla katter med CKD är interstitiell inflammation, tubulär atrofi samt interstitiell fibros.

One of the most common causes of death in domesticated felines is chronic kidney disease (CKD). In patients diagnosed with CKD, the kidneys are damaged to the point that irreversible changes have occurred, causing nephrons to die, by which the capacity of the kidney decreases. The condition mostly occurs amongst elderly cats. Clinical findings include proteinuria, hyperkalemia, hypocalcemia, weight loss and by palpation perceptibly small kidneys.

When diagnosing a patient with CKD the progression of the disease is commonly graded. This is done according to International Renal Interest Society’s scale, which is based on three factors; the level of creatinine in the blood, systolic blood pressure and level of proteinuria. High rates, especially in blood creatinine, suggests well advanced CKD and the possible treatment methods are chosen according to the graded rate.

The aetiology of CKD is not yet clear, however there are several theories. Some of them will be discussed in this literary study. Blood samples, histological examination and anamnesis can give us an idea of the nature of the disease. Microscopic findings in all felines with CKD are interstitial inflammation, tubular atrofia and interstitial fibrosis.
