Abstract

Felin hepatisk lipidos (FHL) är den vanligaste metaboliska leversjukdomen hos katt, och innan behandling med nutritionell support blev standard var sannolikheten för överlevnad låg (ca 5-20%). Sjukdomen innebär en ackumulering av triglycerider i levern vilken leder till minskad leverfunktion och i värsta fall död. Orsaken till att leverlipidos är så pass vanligt hos just katter tros vara att deras unika metabolism gör det svårt för dem att hantera längre perioder av svält. Patogenesen vid FHL är dock inte helt utredd, och syftet med denna litteraturstudie var att sammanfatta de teorier som finns inom området.

De övergripande teorier som finns angående patogenesen vid FHL omfattar en ökad inlagring av triglycerider i hepatocyterna till följd av en minskad betaoxidation i mitokondrier och peroxisomer, en minskad utsöndring av triglycerider i form av VLDL, en ökad nysyntes av fettsyror samt en ökad mobilisering av fett från perifer fettvävnad. Vilka mekanismer som är involverade är däremot föremål för debatt. Betaoxidationen av fettsyror har föreslagits minska vid FHL till följd av ett minskat antal av eller icke-funktionella mitokondrier och peroxisomer, vilket skulle kunna bero på en brist på taurin som behövs för stabiliteten i peroxisomernas membran respektive en brist på långkedjade fleromättade fettsyror (long chain polyunsaturated fatty acids eller LCPUFA´s) som behövs i mitokondriernas membran. Problemet skulle också kunna orsakas av oxidativ stress. Vidare tros transporten av fettsyror in i mitokondrien för betaoxidation minska vid FHL på grund av en brist på L-karnitin. En minskad utsöndring av VLDL från levern tros inte ha någon större betydelse för utvecklingen av FHL då halten triglycerider i blodet är hög vid detta sjukdomstillstånd. Att en ökad mobilisering av fettsyror från perifer fettvävnad bidrar till ackumuleringen av triglycerider i levern stödjs av studier som visat att lipidkompositionen i levern och perifer fettvävnad är mer lika hos katter med FHL än hos friska katter. Nysyntes av fettsyror som bidragande orsak till leverlipidosen kvarstår som teori men dess betydelse har inte kunnat fastslås i några studier.

Det finns en del som fortfarande är outrett angående patogenesen vid FHL. Exempelvis skulle ytterligare studier rörande insulinets betydelse för utvecklandet av FHL behövas, liksom studier där förhållandet mellan n3-LCPUFA’s och n6-LCPUFA’s vid FHL samt den eventuella betydelsen för betaoxidationen och triglyceridinlagringen undersöks. Effekten av vissa kosttillskott, som exempelvis L-karnitin och taurin, vid nutritionell support behöver undersökas för att behandlingen av katter med FHL ska kunna optimeras.

Feline hepatic lipidosis (FHL) is the most common metabolic liver disease in cats, and before treatment using nutritional support was made standard the chance of survival was low (5-20%). The disease involves accumulation of triglycerides in the liver, which leads to decreased liver functionality and potentially death. The reason for hepatic lipidosis being so common in cats is believed to be their unique metabolism, which makes it hard for them to handle longer periods of starvation. The pathogenesis of FHL is not completely clear, and the purpose of this literature study was to summarize the theories that exist within this field of study.

The overarching theories that exist regarding the pathogenesis of FHL comprises an increased inclusion of triglycerides in the hepatocytes caused by a decreased betaoxidation in the mitochondria and peroxisomes, a decreased secretion of triglycerides in the form of VLDL, an increased synthesis of fatty acids and an increased mobilization of fat from the peripheral storages. Which mechanisms that are involved, however, is a matter of some debate. It has been suggested that the betaoxidation of fatty acids decreases in FHL because of a decreased number of, or decreased functionality in, mitochondria and peroxisomes. This could be caused by a lack of taurine, which is needed for the stability of the peroxisomes’ membranes, or a lack of long chain polyunsaturated fatty acids (LCPUFA’s), which are needed in the membranes of the mitochondria. The problem could also be caused by oxidative stress. Furthermore, the transport of fatty acids into the mitochondria for betaoxidation is believed to decrease in FHL due to a lack of L-carnitine. A decreased secretion of VLDL from the liver is not believed to be of any importance in the development of FHL since the amount of triglycerides in blood is high in this disease. That an increased mobilization of fatty acids from peripheral storages contributes to the accumulation of triglycerides in the liver is supported by studies which show that the lipid composition in the two organs is more similar in cats with FHL than in healthy cats. The theory of synthesis of fatty acids as a contributing factor to liver lipidosis remains but its importance has not been proven in any studies.

There are some things that are still uninvestigated regarding the pathogenesis of FHL. For example, further studies regarding the importance of insulin for the development of FHL would be needed, as would studies where the ratio of n3-LCPUFA’s and n6-LCPUFA’s in FHL, as well as its possible effect on betaoxidation and the accumulation of triglycerides, are investigated. The effect of certain supplements, for example L-carnitine and taurine, in nutritional support needs to be investigated so that the treatment of FHL can be optimized
