Abstract

Brakycefala raser blir allt mer populära men deras anatomi kräver stora kunskaper från operationspersonalen för att minimera riskerna för komplikationer under operation. En bristande tillgång till vetenskaplig litteratur inom området lägger ett stort ansvar på djursjukskötaren att anpassa den perioperativa omvårdnaden för en brakycefal patient.

Syftet med kandidatarbetet är att undersöka och jämföra sövningsrutinerna för brakycefala hundar med luftvägsproblem på olika kliniker i Sverige, från ankomst till kliniken till dess att patienten är vaken efter narkosen. En jämförelse mellan sövningsrutinerna för en brakycefal hund med luftvägsproblem och en hund utan luftvägsproblem görs i arbetet. Även en jämförelse mellan fem svenska djursjukhus sövningsrutiner för brakycefala hundar med luftvägsproblem och litteraturens rekommendationer har genomförts.

En telefonintervju utfördes med fem legitimerade djursjukskötare som arbetade på eller ansvarade för operationsavdelningen på sin arbetsplats. Intervjun innehöll 42 frågor. Frågorna gällde djursjukhusets rutiner för sövning av en brakycefal hund med luftvägsproblem och sövningsrutinerna för en hund utan luftvägsproblem.

Litteraturstudien påvisade att brakycefala hundar med luftvägsproblem kräver en större perioperativ omvårdnad. En lugn miljö, preoxigenering, snabb induktion, snabb intubering, specifik placering under uppvaknandet och en försenad extubering var de viktigaste rekommendationerna från litteraturen.

Resultatet visade att kliniker gjorde vissa skillnader i sövningsrutinerna för en brakycefal hund med luftvägsproblem och en hund utan luftvägsproblem. De största skillnaderna i rutinerna mellan en brakycefal hund med luftvägsproblem och en hund utan luftvägsproblem påvisades vara vid preoperativ djurägarinformation, val av läkemedel, preoxygeneringstid och postoperativ omvårdnad. Det var få skillnader på sövningsrutinerna för brakycefala hundar med luftvägsproblem mellan de olika klinikerna som intervjuades, bara val av läkemedel och preoxygeneringstid skilde mellan dem.

Det kunde påvisas liknelser i rutinerna för omvårdnaden av brakycefala hundar med luftvägsproblem mellan de fem intervjuade klinikerna och den vetenskapliga litteraturen. Det fanns dock inte ett samband vid val av läkemedel, varken mellan de olika klinikerna eller i jämförelse med litteraturen. Fler studier inom ämnet önskas för att hitta en optimal sövningsrutin för brakycefala patienter.

Brachycephalic breeds are becoming more popular, but their anatomy requires a great knowledge from the surgery staff to minimize the risks of complications during surgery. A lack of scientific literature in the field places a great responsibility on the veterinary nurse to adapt the perioperative care for a brachycephalic patient.

The aim of this bachelor thesis is to examine and to make a comparison of anesthetic procedures for brachycephalic dogs at different clinics in Sweden, from arrival at the clinic until the patient is awake after anesthesia. A comparison between the anesthetic routines for a brachycephalic dog with respiratory problems and a dog without is discussed in this paper. A comparison between five Swedish animal hospital anesthetic routines for brachycephalic dogs with respiratory problems and scientific literature recommendations has also been conducted.

A telephone interview was conducted with five veterinary nurses who worked in or were responsible for the operation department at their workplace. The interview contained 42 questions. The questions concerned the veterinary clinic’s procedures for anesthesia of a brachycephalic dog with respiratory problems and anesthetic procedures for a dog without respiratory problems.

The literature review showed that brachycephalic dogs with respiratory problems require greater perioperative care. A calm environment, preoxigenation, rapid induction and intubation, specific placement during awakening and delayed extubation were the most important recommendations found in scientific literature.

The result showed some differences in anesthetic procedures for a brachycephalic dog with respiratory problems compared to a dog without respiratory problems. The most obvious differences between treatment of a brachycephalic dog with respiratory problems and a dog without respiratory problems were found to be the preoperative pet owner information, the choice of drugs, the time for pre-oxygenation and the postoperative care. There were few significant differences in the anesthetic routines for brachycephalic dogs with respiratory problems between the different clinics interviewed, only selection of drugs and time for pre-oxygenation differed to some degree.

A correlation between the five interviewed clinics and scientific literature regarding the care of brachycephalic dogs with respiratory problems could be demonstrated. There was, however, no connection in the choice of drugs, either between the different clinics or in comparison with literature. More studies on the subject are desired to find an optimal anesthetic routine for brachycephalic patients.
