Abstract

Efter ortopediska ingrepp så som artroskopi är utformningen av konvalescensperioden viktig för läkningen och återhämtningen. I detta kandidatarbete undersöktes därför svenska hästklinikers konvalescensprogram efter artroskopi av hasled med hjälp av en enkätstudie. Klinikernas hemgångsråd med avseende på postoperativ medicinering, bandagering, stygntagning, boxvila, motion, hagvistelse, rehabiliteringsövningar, utfodringsrutiner, temperaturkontroll samt uppföljning jämfördes sedan med varandra. En litteratursökning gjordes för att studera om det fanns vetenskapligt stöd till klinikernas eftervårdsråd.

Nio enkätsvar samlades in och resultatet tydde på att klinikernas hemgångsråd på många sätt var likartade och till viss del stöttades av vetenskaplig litteratur.

Sju av nio kliniker ordinerade postoperativ analgesi, i samtliga fall NSAID. En utav nio kliniker ordinerade postoperativ antibiotika. Samtliga nio kliniker uppgav att hästarna bandagerades postoperativt, varav de flesta (7/9) rekommenderade bandage under två veckor. Stygntagning rekommenderades av samtliga kliniker tio till 14 dagar postoperativt, helst ett par dagar innan bandaget avlägsnades. De flesta kliniker (6/9) rekommenderade två veckors boxvila. Angående när hagvistelse tilläts var svaren spridda, klinikerna svarade mellan två till tolv veckor, varav de flesta (3/9) rekommenderade efter åtta veckor. Samtliga kliniker rekommenderade skrittmotion för hand under de första två månaderna postoperativt. Rehabiliteringsövningar rekommenderades av tre kliniker, foderrådgivning utav två och daglig temperaturkontroll av åtta.

Klinikernas hemgångsråd liknade i många fall de konvalescensråd som rekommenderades i litteraturen. Däremot saknades vetenskaplig grund till litteraturens rekommendationer om framför allt bandagering, stygntagning, boxvila, hagvistelse och motion. Inom dessa områden saknades studier som stödde konvalescensråden. Rekommendationerna grundades möjligtvis på beprövad erfarenhet eller personliga preferenser hos författarna. Det område inom konvalescens efter artroskopi som var mest studerat var medicinering med avseende på både analgesi och antibiotika. Dessa studier stödde relativt väl klinikernas postoperativa medicineringsordinationer.

Sammanfattningsvis upptäcktes ett samband mellan utbudet av litteratur och samstämmigheten i klinikernas hemgångsråd; ju mer litteratur inom området, desto mer lika var klinikernas svar. Detta tyder på att det behövs mer forskning inom konvalescens och rehabilitering av häst.

Following orthopedic surgeries like arthroscopy, the design of the convalescence period is important for the healing and recovery. Therefore, in this thesis Swedish horse clinics'
convalescence programs following arthroscopy of tarsus was investigated by a questionnaire survey. The clinics' home care advice regarding postoperative medication, bandaging, suture removal, stall rest, exercise, pasture turn-out, rehabilitation exercises, feeding routines, temperature control and follow-up were then compared. A literature
review was conducted to investigate whether the clinics’ after care advice had scientific support or not.

Nine questionnaires were collected and the results showed that the clinics’ after care recommendations in many ways were similar and, to some extent, supported by scientific
literature.

Seven out of nine clinics prescribed postoperative analgesia, in all cases NSAID. One of nine clinics prescribed postoperative antibiotics. All nine clinics reported that the horses were bandaged postoperatively, most of them (7/9) recommended dressings for two weeks. Suture removal was recommended ten to 14 days postoperatively by all clinics, preferably a couple of days before the bandage was removed. Most clinics (6/9) recommended two weeks of stall rest. Regarding when pasture turn-out were recommended, the answers were scattered. The clinics answered between two to twelve weeks, most of them (3/9)
recommended after eight weeks. All clinics recommended walking exercise by hand during the first two months postoperatively. Rehabilitation exercises were recommended
by three clinics, feeding advice by two and daily temperature control by eight.

The clinics' home care advice were in many cases similar the convalescence advice recommended in the literature. However, there was no scientific basis to the literatures’
recommendations, in particular about bandaging, suture removal, stall rest, pasture turn-out and exercise. Within these areas no studies supported the convalescence advice. The literatures’ recommendations were possibly based on proven experience or personal preferences by the authors. The most studied subject within convalescence after
arthroscopy was medication with regard to both analgesia and antibiotics and these studies relatively well supported the clinics postoperative prescription of medication.

In summary, a relationship between the supply of literature and the coherence of the clinics' home care advice were seen; the more literature in the subject, the more similar
answers. This suggests that more research is required within convalescence and rehabilitation of horses.
