Abstract

Prostatasjukdomar är vanligt förekommande hos intakta hanhundar och benign prostatahyperplasi (BPH) är den vanligaste av dem. Multipla studier visar på hög prevalens, upp emot 100 % har rapporterats hos intakta hanhundar redan vid 6 års ålder. Då sjukdomen är vanligt förekommande men kliniskt svår att skilja från övriga prostatalidanden är diagnostiken viktig. För att ställa en säker diagnos krävs vävnadsbiopsi vilket dock är en invasiv metod som till och med är kontraindicerad vid vissa prostatalidanden. Därför är det vanligare att en sannolikhetsdiagnos ställs och det terapeutiska svaret får avgöra om det krävs en omprövning eller inte. I den här studien har en diagnostisk metod där sambandet mellan serumkoncentrationen av ett prostataprotein, CPSE (canine prostatic specific esterase), prostatas utseende samt prostatastorlek, uppmätt via ultraljud, studerats. Analysmetoden som använts är ett relativt nytt kommersiellt test kallat Odelis CPSE (Virbac) som bygger på en ELISA för CPSE.

I studien ingick 49 hundar fördelade på 3 kategorier beroende på klinisk status. Prostatastorleken uppmättes via ultraljudsundersökning och utseendet av prostatas parenkym samt eventuell cystaförekomst noterades. Ett venöst blodprov togs i samband med undersökningen och användes för analys av CPSE-koncentrationen i enlighet med instruktionerna för Odelis CPSE.

Resultaten från studien visar att det inom studiepopulationen fanns stor spridning mellan CPSE-koncentrationerna både inom och mellan de olika grupperna. Statistiskt signifikanta samband sågs mellan prostatastorlek och CPSE-koncentration samt mellan cystaförekomst och CPSE-koncentration medan faktorer som ålder och vikt inte påverkade koncentrationen av CPSE.

Sammanfattningsvis anser författaren att det krävs mer studier för att avgöra den faktiska nyttan av att analysera CPSE som ett diagnostiskt hjälpmedel vid BPH. Analysering av CPSE verkar däremot kunna vara behjälpligt vid diagnostisering av prostatalidanden mer generellt.

Prostatic diseases are common in intact male dogs and benign prostatic hyperplasia (BPH) is the most common of them all. Multiple studies show as high prevalence as 100% in intact male dogs over 6 years of age. Since the disease is common and the clinical appearance is hard to distinguish from multiple other prostatic diseases, the diagnostics are very important. To make a definitive diagnosis it is required to do a tissue biopsy. However, it is an invasive method that is even contraindicated with certain prostate afflictions why it is more common that a probability diagnosis is used instead. In this study a diagnostic method using the correlation between serum concentrations of a prostatic enzyme called canine prostatic specific enzyme (CPSE) and prostatic size (measured by ultrasound) have been evaluated. The analytic method used in the study is a commercial test manufactured by Virbac called Odelis CPSE that uses an ELISA technique.

The study involved 49 dogs divided into 3 categories depending on their clinical status. Prostate size was measured by ultrasound and the appearance of the prostatic parenchyma and possible cyst occurrence was noted. A venous blood sample was taken and serum was used for the analysis of the CPSE concentration in accordance with the instructions for Odelis CPSE.

The results of the study show that there was a large spread in CPSE concentrations both in, and between, the client groups. Statistically significant associations were found between prostate size and CPSE concentration and between presence of cysts and CPSE concentration while factors such as age and weight did not affect the concentration of CPSE.

The author is not fully convinced of the benefits of analysing CPSE as a diagnostic tool for BPH until further studies have been made, but sees the potential for it as a more general indicator for prostatic complaints.
