Abstract

Antalet geriatriska hundar som genomgår anestesi ökar i takt med att djursjukvården utvecklas och blir mer avancerad. Syftet med arbetet är att genom en litteraturstudie sammanställa befintlig kunskap om åldersrelaterade fysiologiska förändringar hos hundar och dess effekter vid anestesi, vilka omvårdnadsbehov geriatriska hundar har under anestesi samt vilka faktorer som bör beaktas vid anpassning av anestesin efter den geriatriska hundens hälsotillstånd.
Risken för anestesirelaterade komplikationer och mortalitet är ökad hos geriatriska hundar vilket kan bero på åldersrelaterade fysiologiska förändringar som leder till en minskad reservkapacitet i organsystemen. Åldersrelaterade förändringar som bör beaktas under anestesi är bland annat en ökad känslighet för anestetika, en nedsatt läkemedelsmetabolism, en försämrad blodtrycksreglering, en ökad risk för hjärtarytmier, nedsatt gasutbyte i lungorna samt en försämrad termoreglering. Dessa förändringar ökar risken för komplikationer under anestesi som exempelvis hypotension, hjärtarytmier, hypotermi, hypoxemi, förlängd återhämtning och stress.
En preanestetisk bedömning kan användas som underlag för anpassning av anestesimetod och val av läkemedel för anestesi av geriatriska hundar. Läkemedel bör väljas med hänsyn till den försämrade kardiorespiratoriska kapaciteten och den försämrade kapaciteten för läkemedelsmetabolism. Den perianestetiska omvårdnaden bör syfta till att förebygga utveckling av komplikationer och kan bestå av extern värmetillförsel för att hindra hypotermi, vätsketerapi för att främja en stabil cirkulation samt omsorgsfull intraoperativ positionering för att undvika smärta. Då åldersrelaterade fysiologiska förändringarna ha betydande effekter på komplikationsrisken under anestesi är det viktigt att anestesin och den perianestetiska omvårdnaden anpassas efter den geriatriska hundens individuella hälsotillstånd för att öka säkerheten vid anestesi av geriatriska hundar.

The number of geriatric dogs undergoing anesthesia increases as veterinary medicine and animal healthcare develops and becomes more advanced. The purpose of this study is to summarize existing knowledge through a literature review about age-related physiological changes in dogs and their effects during anesthesia, the nursing requirements of geriatric dogs undergoing anesthesia and what factors to consider when adapting anesthesia to the health condition of the geriatric dog.
The risk of anesthesia-related complications and mortality is increased in geriatric dogs which may be due to age-related physiological changes that cause a reduction in reserve capacity of organ systems. Age-related changes that are important to consider during anesthesia include an increased sensitivity to anesthetics, decreased metabolism and excretion of drugs in the liver and kidneys, impaired regulation of blood pressure, an increased risk of cardiac arrhythmias, impaired gas exchange in the lungs and an impaired thermoregulation. The age-related physiological changes increase the risk of a number of complications during anesthesia, such as hypotension, dehydration, fluid overload, cardiac arrhythmias, hypothermia, hypoxemia, prolonged recovery and stress.
A preanesthetic assessment can be used as a basis for the adaptation of anesthetic method and choice of drugs for anesthesia in geriatric dogs. Medications should be selected in regard to the decreased cardiorespiratory capacity and the reduced capacity for drug metabolism. The perianesthetic nursing care should aim to prevent the development of complications and may consist of provision of external heat sources to prevent hypothermia, fluid therapy to facilitate a stable circulation and careful intraoperative positioning to avoid pain. Since age-related physiological changes may have significant effects under anesthesia and may affect the risk of anesthesia-related complications, anesthesia and the perianesthetic nursing care should be adapted to the geriatric dog's individual condition in order to increase safety for geriatric dogs under anesthesia.
