Abstract

Denna litteraturstudie behandlar strålbenshälta hos häst, även känd som Navicular disease (ND). Studien beskriver sjukdomen och dess karaktäristiska drag, samt belyser de två huvudsakliga teorierna om den bakomliggande orsaken till insjuknandet. Det finns tydliga inklusions- och exklusionskriterier, vilka är viktiga för att kunna göra en omfattande hältutredning och för att kunna ställa rätt diagnos, som är betydande för att ge rätt behandling. Litteraturstudien belyser även det faktum att hästar drabbade av ND har visat sig ha högre intramedullärt tryck i strålbenet, samt vilka genetiska riskfaktorer som visat sig vara kopplade till sjukdomen. De genetiska defekter som behandlas är röntgenförändringar i strålbenskonturen (RAC) och strålbensstrukturen (RAS), samt utvecklandet av deformerade näringsfåror i strålbenet (DCS). Med hjälp av helgenomskanning av arvsmassan har sk ”Quantitative trait loci” (QTL) associerade till sjukdomen kunnat identifieras på sammanlagt 9 olika kromosomer. Detta möjliggjorde att en mikrosatellit med signifikant koppling till sjukdomen hittades och visade sig vara belägen inom en region synteniskt kopplad till artros hos människa. Fokus lades därefter på DCS, där genotypisk association i VSTM1-genen (V-set transmembrane domain containing 1) har hittats, vilken är kopplad till osteoklaster som bryter ned ben. Samtidigt hittades också allelisk association för en SNP (Single Nucleotide Polymorphism) inom IRF3-genen (interferon regulatory factor 3), vilken medierar transduktionsprocesser i reumatoid artrit. Det faktum att IRF3-genen är en riskfaktor för ND är en mycket stark indikation på att det finns en im-munologisk komponent vid sjukdomsförloppet. ND är en komplex sjukdom som kräver om-fattande hältutredningar, och desto djupare förståelse om de genetiska aspekterna av sjukdomen och dess tillhörande riskfaktorer, desto säkrare avel kan tillämpas. I grund och botten är varje djursjukdom en djurvälfärdsfråga, central för både djur och människor.

This literature study deals with Navicular disease (ND) in horses. The study gives a description of the disease and its characteristic features. It also highlights the main theories about the underlying causes, and in order to make an accurate diagnosis inclusion- and exclusion criteria have been defined. These criteria play a major role, in order to make an accurate lameness examination and to make a proper diagnosis, which are crucial for right treatment to be given. The literature study also considers the fact that horses who are diagnosed with ND seems to have a higher intramedullar pressure than healthy horses, and highlights what genetic risk factors have been found to be associated with the disease. The genetic defects mentioned in the study are radiological alterations in the contour (RAC) and radiological alterations in the structure (RAS) of the navicular bone, and also the development of deformed canales sesa-moidales (DCS). With whole genome scanning, QTLs associated with the disease has been found on 9 equine chromosomes (ECA). This made it possible to find a microsatellite with significant association to the disease which appeared to be located within a region syntenic to human osteoarthritis. Thereafter, focus was put on DCS, where genetic association to the VSTM1 gene (V-set transmembrane domain containing 1) was found, which is linked to bone degrading osteoclasts. At the same time, allelic association for a SNP (Single Nucleotide Polymorphism) within the IRF3 gene (interferon regulatory factor 3) was found, which mediates transduction processes in rheumatoid arthritis. The genetic association to the IRF3 gene is a very strong indication that there is an immunologic component of the disease. ND is a com-plex disease, requiring extensive lameness examinations, and the deeper understanding of the genetic aspects of the disease and its associated risk factors, the safer and healthier breeding can be applied. Basically, each animal disease is an animal welfare issue, central to both animals and humans.
