Abstract

Hos häst finns två olika arter av spolmask, Parascaris equorum och Parascaris univalens. P. equorum har ansetts vara den av parasiterna som orsakar infektion hos häst. Huruvida väl detta stämmer är dock en fråga som nyligen lyfts fram och är betydelsefull för framtida forskning. Den här litteraturstudien kommer ändå huvudsakligen inrikta sig på P. equorum då tidigare studier som gjorts utgår ifrån denna art.
Resistensutvecklingen hos P. equorum har skett i en oroväckande hög hastighet i flera länder världen över, en inbromsning av utvecklingen är av stor betydelse. Att påvisa resistens mot ett anthelmintika i ett tidigt stadium är mycket viktigt då det tros spela en roll för hur väl resistensutvecklingen kan bromsas. Det finns flera faktorer som samverkar då resistens utvecklas, men i vilken mån de olika faktorerna spelar in är inte helt klarlagt. Vilka anthelmintika som det finns resistens mot tycks variera beroende på land, resistensen mot makrocykliska laktoner troddes tidigare vara utvecklad överallt. Nya studier har visat att makrocykliska laktoner fortfarande har effekt på vissa håll i Danmark. I Sverige har resistens ännu inte påvisats mot bensimidazoler eller pyrantel.
Många föl klarar sig igenom en infektion med P. equorum utan att uppvisa några symptom och tillförskansar sig därigenom en förvärvad immunitet mot parasiten. Vid kraftig parasitbörda kan kolik och tarmobstruktion uppstå, vilket i värsta fall kan leda till enterit och perforering av tarmväggen med akut peritonit, buksmärta och hastig död.
Antalet parasitägg i hästens avföring kan mätas och användas för att konstatera en infektion. Något samband mellan antalet ägg och antalet vuxna P. equorum som hästen bär på finns därmot inte. Trots detta är det vid selektiv behandling endast de hästar som utsöndrar ett visst antal ägg som behandlas med anthelmintika. Selektiv behandling rekommenderas exempelvis vid behandling av cyathostomer hos vuxna hästar då det tros minska parasitens resistensutveckling. För P. equorum rekommenderas det i dag två behandlingar under fölets första 16 veckor och ytterligare en behandling efter påvisad infektion vid ca 10 månaders ålder. Det finns även andra parasitkontrollerande åtgärder, som beteshygien, vilka bör kombineras med anthelmintikabehandling för att hålla parasittrycket ner och minska resistensutvecklingen, dessa metoder är dock ofta tidskrävande för hästägaren.

In horses there are two different species of roundworms, Parascaris equorum and Parascaris univalens. P. equorum has been considered the parasites that cause infections in horses, but recent studies has shown that P. univalens occurs more frequently. Whether this is true or not has recently been highlighted and could be an issue of importance for future research. This literature review will mainly focus on P. equorum since previous studies are based on this species.
The development of resistance in P. equorum has according to various studies occurred at an alarming high rate in several countries worldwide. A reduction of the development is of great importance. The detection of resistance against an antihelmintic in an early stage is important as it is believed to play a role in slowing down the development of resistance. There are several factors that influence the development of the resistance but the vastness of each of the different factors is not entirely clear. Resistance against different antihelmintics seems to vary depending on the country. Resistance against macrocyclic lactones were previously thought to be developed everywhere, but recent studies have shown that macrocyclic lactones still have effect in Denmark. In Sweden resistance have not yet been developed against benzimidazoles or pyrantel.
Many healthy foals make it through infection with P. equorum without showing any symptoms and will by age obtain an acquired immunity against the parasite. In some cases infection can cause symptoms as distress to the animal, and in worst case lead to death due to obstruction and rupture of the small intestine.
The number of parasite eggs in horse faeces can be measured and used to establish an infection but no correlation between the number of eggs and the number of adult P. equorum in the horse has been established. Despite this, in selective treatment it is only those horses that secrete a certain number of eggs that is treated with antihelmintic. Such selective treatment recommended for the treatment of cyathostomes in adult horses as it is believed to reduce the development of parasitic resistance. Today the recommended treatment for P. equorum is two treatments during the foals first 16 weeks and an additional treatment after proven infection at about 10 months of age. There are also other methods to control the parasite burden, such as pasture hygiene, which should be combined with antihelmintic treatment to keep the parasitic pressure down and to reduce the development of resistance, however these methods is often time consuming for the horse owner.
