Abstract

Rabiesvirus är en allvarlig sjukdom globalt då minst 35 000 människor mister livet varje år på grund av den fatala sjukdomen. Idag finns inget botemedel mot rabiesvirus när sjukdomen brutit ut, det enda skyddet är vaccinering. Hur immunförsvaret reagerar på viruset, varför inte immunförsvaret klarar av att eliminera viruset och vad post-vaccinering innebär undersöks i det här arbetet.
Rabiesvirus utsöndras i saliv och den vanligaste infektionsvägen för rabiesvirus är via bett från infekterad individ och för människa är hundbett största risken. När infektion har skett transporteras virus till centrala nervsystemet (CNS) via intraaxonal transport. Rabiesvirus är ett neurotropt virus och replikerar i CNS. Huruvida replikation sker innan viruset når CNS är ej klarlagt. Inkubationstiden varierar mellan individer, djurslag och infektionsvävnad, dock kan virusutsöndring ske en kort tid innan symptom uppkommer. Det finns två typer av symptombilder vid rabiesinfektion, en furiös och en paralytisk.
Immunförsvaret vid en rabiesinfektion består framförallt av ett interferon (IFN) svar som är direkt antiviralt och ger upphov till apoptos av infekterade celler. Perifert uppnås ett antikroppssvar men även ett cellmedierat svar där cytotoxiska T-celler har en viktig funktion. Dock undviker rabiesvirus immunförsvaret på flera sätt. Inget immunförsvar uppnås innan viruset börjat replikera i CNS och då finns blod-hjärnbarriären som ett hinder för immunförsvaret. Att inget immunsvar uppnås innan denna punkt i infektionen kan bero på den låga infektionsdos som krävs. Det typiska svaret att låta virusinfekterade celler genomgå apoptos undviker rabiesvirus genom att reglera nivåer av sitt glykoprotein. IFN inhiberas via rabiesvirus fosfoprotein. Apoptos av infekterade neuron undviks genom skapande av Negrikroppar som består av virusprotein och receptorn TLR3. Antikroppssvaret klarar ej av att eliminera rabiesvirus då det inte hinner nå CNS på grund av blod-hjärnbarriären. Det cellmedierade svaret undviks genom att rabiesvirus ger en uppreglering av FasL och B7-H1, vilket leder till T-cellsapoptos.
Det finns vaccin att tillgå för både människor och djur, men det är bara för människor som post-vaccinering är utvecklat. Post-vaccinering, det vill säga vaccinering efter exponering, är nära 100 % effektivt om det ges innan rabiesviruset når CNS. Behandlingen innebär inte bara vaccination utan även en grundlig sårrengöring, behandling med virusneutraliserande antikroppar (VNA) och en aktiv immunisering.
Rabiesvirus har många metoder att klara sig undan ett gott fungerande immunförsvar och vaccinering är därför en bra metod för att stoppa virusspridningen. Rabies utrotades i stora delar av västra Europa genom distribuering av vaccin i bete till vilda djur. Fortsatta försök att kontrollera spridningen i den vilda populationen görs då detta tros vara viset att stoppa spridningen helt.

Rabies virus is the cause of serious disease worldwide, at least 35 000 people die every year because of this fatal disease. At the moment there is no medication available and after the symptoms of disease appears no treatment exists. Available vaccines function as prophylaxis and post-exposure prophylaxis, as long as the virus has not reached the central nervous system (CNS). How the immune system reacts to rabies virus, why the immune system cannot eliminate the virus and how post-exposure prophylaxis work are examined in this work.
Rabies virus is shed in saliva and the most common route of infection is by a bite from an infected animal and when it comes to human infection, it is due to dog bites. After infection the virus is rapidly transported to CNS through intra axonal transport. Rabies virus is a neurotropic virus that replicates in the CNS, but it is not known whether the virus replicates before it reaches the CNS or not. The incubation time varies between individuals, species and type of infected tissue. Virus shedding can however occur before the onset of symptoms. Two kinds of rabies forms are known, furious rabies and paralytic rabies.
The immune response to rabies virus should be initiated by interferon (IFN) production, which has an antiviral effect and results in apoptosis of infected cells. In the periphery an antibody and a cell-mediated response should be achieved. However, rabies virus escapes the immune system in several ways. There is no immune response until the virus replicates in the CNS. The blood-brain barrier makes it difficult for the immune system to reach the virus. Typical immune response to virus infections is that infected cells undergo apoptosis. Rabies virus is able to avoid this by regulating the amount of its Glyko-protein that is synthesized. Apoptosis of infected neurons is also avoided by the formation of Negribodies that consist of viral protein and the receptor TLR3. The effects of IFN is inhibited by the virus Phospho-protein. The antibody response that develops is not effective enough to eliminate the virus. This is due to the resistance of the blood-brain barrier. The cell-mediated response is avoided through upregulation of FasL and B7-H1 by the virus, which results in death of T-cells.
Vaccines both for humans and animals are available, but post-exposure prophylaxis (PEP) is available only for humans. Post-vaccination is almost 100 % effective if administrated before the virus reaches the CNS. PEP include proper wound cleaning, treatment with virus neutralizing antibodies (VNA) and active immunization.
Rabies virus uses several methods to avoid elimination by a perfectly working immune system. Vaccination is a good way to prevent virus from spreading. Rabies virus is close to extinction in parts of western Europe as a result of distribution of vaccine in baits to wild animals. Ongoing work to control the spread in the free population is believed to be a good way to control or even eradicate the rabies virus completely.
