Abstract

Topikal administrering av atropin är en viktig del i behandlingen av ulcerativ keratit och främre uveit hos häst. Atropin har flera viktiga egenskaper vid lokal behandling av ögat såsom förmågan att framkalla pupilldilatation, paralys av ciliarmuskulaturen samt minska läckaget av proteiner och celler ur inflammerade kärl i iris. Sammantaget minskar atropin därmed risken för synechier mellan närliggande strukturer i ögat och genom detta minskar risken för komplikationer som sekundär katarakt och glaukom, samt synpåverkan till följd av en pupill som inte kan röra sig normalt. Att framkalla pupilldilatation samt cykloplegi minskar dessutom den smärta som är associerad med mios och spasm i ciliarmuskulaturen.
Inom veterinärmedicinen har det under en längre tid diskuterats kring de gastrointestinala biverkningar som kan förekomma vid behandling med atropin hos häst. Det finns några få studier publicerade som visar på sänkt tarmmotilitet vid administrering av atropin. Bland annat har Williams et al. (2000) publicerat en studie som visade på kraftigt sänkt tarmmotilitet, samt ökad risk för kolik vid användning av mycket höga doser topikalt administrerat atropin hos häst. Detta har lett till viss försiktighet i användandet av höga doser atropin i form av ögondroppar. I den experimentella delen av detta examensarbete har vi därför valt att undersöka de systemiska effekterna, framförallt med avseende på tarmmotilitet och koliksymptom, vid topikal aministrering av atropin i de doser som idag rekommenderas i klinisk verksamhet vid behandling av uveit hos häst (Gilger et al., 2011).
Resultatet av denna studie visar att topikal administrering av 0,1 ml atropin (1 %) var tredje timme under två dygn och därefter två gånger dagligen i ytterligare tre dygn ger signifikant nedsatta auskulterbara tarmljud i jämförelse med administrering av placebo. Däremot har vi inte kunnat se någon ökad risk för kolik då ingen av hästarna vid något tillfälle under försökets gång uppvisade koliksymptom.

The use of topical atropine plays an important role in the treatment of ulcerative keratitis and anterior uveitis in horses. Atropine has some important properties when instilled topically onto the eye including the ability to induce pupillary dilation, paralysis of the ciliary body musculature and reduce the leakage of cells and proteinaceous fluid from inflamed iridal vessels. Altogether, this reduces the incidence of synechia formation and consequently reduces the risk for secondary cataracts and glaucoma and loss of vision due to a pupil that is unable to move properly. Inducing pupillary dilation and cycloplegia also reduce the pain associated with miosis and spasm of the ciliary body musculature.
In veterinary medicine, it is known that gastrointestinal side effects can occur after administration of atropine in horses. There are a few published studies regarding this subject. For example Williams et al. (2000) published an article showing that very high doses of topically administered atropine markedly reduced gut motility and induced colic in horses. This has led to some precaution as to the use of high doses of atropine in this species. In the experimental part of this study, we therefore try to evaluate the systemic effects, particularly effects on gut motility and colic signs, of topical administration of atropine in doses that are commonly used in clinical practice in the treatment of moderate to severe uveitis (Gilger et al., 2011).
Our results show that topically administered atropine (1%) 0.1 ml every three hours during two days followed by 0.1 ml two times a day reduce the auscultated gut sounds in comparison to administration of placebo. However, we did not find an increased risk of colic, while none of the horses at any point during the clinical trial exhibited signs of colic.
