Abstract

Njurarna är två viktiga multifunktionella organ och alla sjukdomar som ger njurpåverkan kan resultera i nedsatt njurfunktion. Hur stort bortfall av njurfunktionen som åsamkats har betydelse för individens välmående och överlevnad, varför det är viktigt att tidigt upptäcka och behandla sjukdomen.
Ärftliga njursjukdomar är i de flesta fall progressiva och i slutändan fatala. En ökad medvetenhet och kunskap om ärftliga njursjukdomar kan genom möjligheterna till ett förbättrat avelsarbete bidra till minskat lidande och ökad välfärd hos individerna i drabbade hundraser.
I en ännu opublicerad svensk studie identifieras bl.a. raserna shetland sheepdog (SHS) och cavalier king charles spaniel (CKCS) som två av raserna med högst incidens av njursjukdom. Dessa två raser omnämns dock sällan som särskilt drabbade i den veterinärmedicinska litteraturen.
Examensarbetets syfte var att utifrån journaler från två stora svenska djursjukhus (Evidensia Södra Djursjukhuset och Universitetsdjursjukhuset, Sveriges Lantbruksuniversitet) beskriva vilka njursjukdomar som diagnosticerats hos dessa raser samt undersöka om någon sjukdom verkade förekomma mer frekvent.
Genom sökningar på njursjukdomsspecifika diagnoskoder i djursjukhusens journaldatabaser så återfanns de individer som senare inkluderades i studien. Utvalda journaler studerades och hundarna grupperades, inom respektive ras, med avseende på kön, ålder vid symtomdebut samt använd diagnoskod. Genom att studera journaltexten identifierades, i de fall det var möjligt, även den mer specifika diagnos som ställts på respektive patient, vilken ibland skiljde sig betydligt från använd diagnoskod.
Resultatet av studien var att diagnostiken avseende njursjukdomar var mycket ospecifik hos både SHS och CKCS samt att inga indikationer sågs för att någon specifik njursjukdom skulle vara överrepresenterad. Däremot sågs en indikation för att den medfödda, ibland ärftliga, njursjukdomen renal dysplasi skulle kunna finnas hos båda raser, men för att säkerställa detta krävs vidare studier.

The kidneys are two important and multifunctional organs. All diseases that may affect the kidneys can cause a loss of kidney function. The extent of kidney function loss will have an impact on the animals’ health and survival. An early recognition is therefore crucial.
Inherited kidney diseases are in most cases progressive and ultimately fatal. An increased knowledge and awareness of hereditary kidney diseases might contribute to a diminished suffering and increased welfare for dogs in affected breeds, through the possibilities of performing improved breeding programs.
In a (to date unpublished) Swedish study, Shetland Sheepdogs (SHS) and Cavalier King Charles Spaniel (CKCS) were identified as two of the dog breeds with the highest incidence
of kidney disease in a large population of Swedish insured dogs. Neither one of these breeds are commonly mentioned as particularly affected by kidney diseases in the literature of veterinary medicine.
The purpose of this study was to try to identify which kidney diseases that had been diagnosed in these two dog breeds based on the medical records from two big Swedish animal hospitals (Evidensia Södra Djursjukhuset and Universitetsdjursjukhuset, Swedish University of Agricultural Science), and to investigate if any particular disease seemed to appear more often.
The dogs included in this study were found through searches for kidney disease specific diagnostic codes in the hospital’s medical record data bases. The selected medical records were carefully scrutinized and the dogs were grouped, within the breeds, for gender, age when signs first were noticed and the diagnostic code used. By studying the medical records further, in some cases, a more specific kidney disease was diagnosed, which sometimes differed considerably from the diagnostic code used.
The results of the study were that no specific etiology of kidney disease was diagnosed more often than others in neither SHS nor CKCS dogs. Renal dysplasia was diagnosed in dogs of both breeds. Further studies are needed to investigate if renal dysplasia is a common problem in these breeds.
