Abstract

Kolik är ett vanligt sjukdomskomplex hos hästar som i vissa fall kan kräva kirurgisk behandling. Kirurgisk behandling är dyrt och komplicerat varför många studier har gjorts för att försöka förklara vad som påverkar prognosen efter buköppning. Resultaten i de olika studierna skiljer sig åt i många fall, vilket gör det intressant att undersöka hur prognosen ser ut för de hästar som genomgår kirurgisk behandling av kolik vid Universitetsdjursjukhuset (UDS) i Uppsala. Syftet med studien var att undersöka och beskriva prognosen för hästar som genomgått bukkirurgi på UDS samt att undersöka hästägarnas inställning till buköppning. Studien är en retrospektiv studie kombinerat med en telefonintervju med ägarna till inkluderade hästar.
Av de hästar som togs till operation överlevde 60.2 % så att de skickades hem, överlevnad fram till hemgång räknat på de hästar som överlevde operation och uppvak var 81.5 %. Ett år efter operationen så levde fortfarande 71.4 % av hästarna som skickades hem efter operationen. Endast en häst kom inte tillbaks till tidigare användning på grund av problem som eventuellt kunde relateras till bukkirurgin, många av hästarna kom tillbaks till tidigare användningsområde inom sex månader. Hästägarna är generellt positiva till kirurgisk behandling av kolik men tycker det blir mer tveksamt på äldre hästar, de ansåg framför allt att tiden på kliniken och oro över sårkomplikationer och fler kolikanfall efter hemgång var jobbigt.
Överlevnaden bland hästarna som tas till operation varierar mellan olika studier och djursjukhus vilket troligtvis beror på olika kriterier i beslutet mellan att avliva, behandla medicinskt eller kirurgiskt. Överlevnaden till hemgång bland de hästar som överlever operation och uppvak varierar däremot lite mellan olika studier. I denna studie sågs en hög återgång till tidigare användning, det är dock svårt att jämföra dessa resultat med andra studier då ingen liktydig definition av ”användning” eller ”prestation” finns. Hästägarna är positivt inställda till buköppning men då det enbart är ägare till hästar som har överlevt sin buköppning som intervjuats bör man räkna med att det har påverkat resultatet.
En studie där även ägare vars hästar har avlivats vid operationen samt de hästägare som avstod operation intervjuas skulle vara intressant för att få större förståelse för hästägarnas inställning till buköppning.

Colic is not an uncommon disease in horses and surgical treatment is sometimes required. Such treatment in horses is expensive and complicated and many studies have been designed in order to evaluate what factors that affect the prognosis. The results differ widely depending on clinics and design of study and it is therefore difficult as a veterinarian working in Sweden to draw conclusions applicable to one’s own workplace. It therefor makes it interesting to find out the prognosis for horses undergoing surgery for colic at UDS (University Animal Hospital). The aim of this study was to asses and describe the prognosis for horses undergoing surgical treatment of colic at UDS and to find out what attitude the horse-owners had towards such procedures. This is a retrospective study combined with a telephone interview with the owners to horses included in the study.
Sixty % of horses undergoing surgical treatment of colic at UDS survived the procedure, recovered from the anesthesia and were discharged from the clinic. Eighty one % of horses that recovered from anesthesia survived to discharge and 71.4 % of horses that were discharged from UDS were still alive one year post- surgery. The majority of the horses were able to return to the same level of performance that they were at pre-surgery within six months post-surgery. Only one horse failed to return to previous level of performance, possibly due to reasons related to the surgical treatment of the colic at UDS. Horse-owners generally have a positive attitude towards surgical treatment of colic, but they are more hesitant if the horses are geriatric. Time spent at the clinic (UDS), worry of complications of the incision wound and recurrence of colic episodes after discharge from the hospital were the main concerns’ of the horse owners.
The survival rate of horses undergoing surgical treatment for colic differ from study to study and also between hospitals. This is likely due to variation of the criteria of when to decide for euthanasia, and the criteria for when surgical- or medical treatment of the colic is the best option. The survival rate of horses surviving both the surgery as well as the recovery from anesthesia does not differ much when comparing different studies. The present study found a high rate of horses that returned to previous level of performance after treatment at UDS. It is however difficult to compare these results with other studies because there is no comparable definition neither to level of performance nor for what the horses were used for. The overall positive opinion among the horse owners towards surgical treatment of colic might be due to that only owners whose horses survived their surgery were interviewed in this study.
For future references, it would be interesting to conduct a study where the owners of horses that were euthanized during surgery and owners that chose not to treat their horse surgically were interviewed about their opinion and feelings towards surgical treatment of colic.
