Abstract

Kolik kan vara ett allvarligt tillstånd hos hästar och omfattar all smärta från buken. Hästarna visar detta symptom på olika sätt, exempelvis genom att rulla eller sparka med benen mot buken. Orsaken till kolik varierar och kan till exempel vara parasitangrepp, tvära foderbyten eller brist på motion.
NSAID, non-steroidal antiinflammatory drugs, är en grupp läkemedel som används internationellt för att behandla smärta, inflammation och feber. NSAID verkar genom att inhibera enzymet cyklooxygenas (COX) och därmed hämmas produktionen av prostaglandiner – vilket ger de smärtlindrande, antiinflammatoriska och febernedsättande egenskaperna. Dock ger dessa läkemedel också en risk för biverkningar då prostaglandiner även har viktiga fysiologiska funktioner, bland annat att skydda mag-tarmslemhinnan. COX finns i två isoformer, COX-1 och COX-2. De traditionella icke selektiva NSAID inhiberar båda isoformerna, men under 1990-talet utvecklades så kallade coxiber som selektivt inhiberar COX-2. COX-1 påstås ha en större roll än COX-2 för att upprätthålla normalfunktion i mag-tarmslemhinnan och därför anses coxiberna minska biverkningarna på mag-tarmkanalen.
Syftet med litteraturstudien är att undersöka om det finns evidens för att de COX-2 selektiva coxiberna har fördelar framför icke selektiva NSAID vid smärtlindring av koliktillstånd på häst och belysa eventuella biverkningar på mag-tarmkanalen.
Det finns relativt få kliniska studier tillgängliga med syfte att undersöka biverkningarna av NSAID på mag-tarmkanalen hos hästar. Dock redovisar flera experimentella studier att icke selektiva NSAID (flunixin) har en negativ påverkan på läkningsprocessen av ischemiskt skadad tarm, jämfört med obehandlade kontrollgrupper och en grupp behandlade med COX-2 selektiva NSAID (firocoxib). Det finns viss evidens i litteraturen att rekommenderade doser av icke selektiva NSAID (fenylbutazon och meloxicam) inte ger någon signifikant ökad förekomst av magsår vid kortvarig behandling av i övrigt friska hästar. Dock visar en studie en signifikant ökad grad av magsår vid en kombinerad behandling med både fenylbutazon och flunixin. Även långtidsbehandling med fenylbutazon har visat sig ge uppkomst av magsår. Vid behandling med meloxicam i högre doser än av tillverkaren rekommenderat har biverkningar noterats i experimentella studier. Två av fem hästar i en studie utvecklat höger dorsal kolit av de som fick 5 gånger högre dos än rekommenderat.
Det finns en antydan i litteraturen att coxiberna inte har lika stor negativ påverkan på läkningen av ischemisk tarm som vissa icke selektiva NSAID har. Fler kliniska och experimentella studier, framför allt jämförande mellan coxiber och icke selektiva NSAID, behövs för att kunna jämföra de bådas egenskaper och dra slutsatser med större säkerhet.

Colic describes abdominal pain and can be a serious condition in horses. The horse can present symptoms differently for example rolling and kicking. Causes of colic can be many for example parasites, lack of exercise or too quick changes in diet.
Non-steroidal anti-inflammatory drugs (NSAID) are a group of substances used worldwide to treat pain, inflammation and fever. The mechanism of action is to inhibit the enzyme cyklooxygenas (COX) and consequently the production of prostaglandins, which result in the pain-relieving, anti-inflammatory and antipyretic effects. The inhibition of prostaglandins is also associated with side effects, because of the importance of prostaglandins in the physiological function of the mucus membrane in the stomach and the intestines. Two isoforms of COX exist, COX-1 and COX-2. NSAIDs can be either the traditional unselective NSAID that inhibit the both isoforms or selective more novel coxibs that were developed under the 1990s to selectively inhibit COX-2. The hypothesis is that COX-1 is more important than COX-2 in the protection of the gastro intestinal mucosa, and therefore the coxibs should be associated with fewer side effects than the traditional NSAID.
The purpose of this literature study is to evaluate if coxibs are more favorable compared with traditional NSAIDs when treating colic in horses, with focus on gastro-intestinal side effects.
Some studies indicate that the non-selective NSAID (flunixin) has a negative effect on the healing process of the ischemic gut, compared to a placebo and to a group treated with the selective NSAID (firocoxib). Ulcers caused by NSAID have also been studied. Recommended doses of unselective NSAID (fenylbutazone and meloxicam) do not appear to give any significant increase in ulcer-score in short-term treatment. A combined treatment using both fenylbutazone and flunixin resulted in increased ulcer-score. Side effects have been shown when horses were treated with meloxicam in doses higher than recommended. When given 5 times larger dose than recommended, 2 out of 5 horses got right dorsal colitis.
More experimental/clinical studies are required to conclude if coxibs are associated with fewer side effects than the traditional NSAIDs. However there is limited evidence from experimental studies that coxibs may have less negative effects on the healing of ischemic gut than some of the non-selective NSAIDs seem to have.
