Abstract

Hälta är idag den vanligaste anledningen till sjukdom hos svenska hästar och den vanligaste orsaken till hälta är osteoartrit (OA). Ställandet av diagnosen OA kommer ofta för sent så att irreversibla ledskador redan hunnit uppstå vilket gör det av yttersta betydelse att förbättra
diagnostiken inom området. OA kan uppstå på flera olika sätt; akut trauma mot leden, intraartikulära frakturer, repetitivt trauma, skada på mjukdelar som orsakar en instabilitet i leden med flera. Förenklat kan OA orsakas till följd av en normal belastning på en onormal
led eller onormal belastning på en normal led. Oavsett orsak ses skador på ledbrosket och beroende på hur utvecklad OA är kan röntgenförändringar ses, vilka avspeglar ett dåligt broskstatus. OA kan betraktas som en inflammatorisk ledsjukdom där flera ledkomponenter
bidrar till inflammationen, bland annat producerar både synoviocyterna och kondrocyterna inflammationsmediatorer.
I sökandet efter en bättre diagnostik för OA undersöks möjligheten att med hjälp av biomarkörer för inflammatoriska och vävnadsnedbrytande processer i synovia och blod erhålla en tidig och korrekt diagnostik. I den här studien undersöktes med hjälp av ämnesspecifika ELISA cytokinerna interleukin (IL)-1β, high mobility group box protein (HMGB)-1 och enzymet matrix metalloproteinas (MMP)-13, vilka alla kan vara av betydelse i sjukdomsprocessen vid OA. Hypoteserna för arbetet var att förhöjda koncentrationer av IL-1β, HMGB-1 och aktivt MMP-13 kan ses i synovia och serum vid förekomst av olika typer av
patologiska ledförändringar och ju gravare makroskopiska patologiska ledförändringar desto högre nivåer av IL-1β, HMGB-1 och aktivt MMP-13 kommer att kunna påvisas i synovian.

Vidare var hypotesen att nivåerna av IL-1β, HMGB-1 och aktivt MMP-13 i synovia och serum kommer att vara korrelerade. Fjorton patienthästar ingick i studien och sammanlagt erhölls synovia från 24 kot- och
karpalleder i samband med artroskopi. Koncentrationer av IL-1β, HMGB-1 och aktivt MMP-13 analyserades i synovia och blod med hjälp av ELISA och förekomst av typ och grad av
ledskador noterades under artroskopi. Synovian analyserades även med avseende på antal leukocyter. Aktivt MMP-13 kunde inte detekteras i några patientprover och IL-1β
detekterades bara i prov från en häst. HMGB-1 detekterades i 20 synoviaprover och 12 serumprover, och koncentrationen av HMGB-1 i synovia korrelerade signifikant med antal
leukocyter i synovia. Koncentrationen av HMGB-1 i synovia var ofta högre om synovit eller fraktur påvisades makroskopiskt, men statistisk signifikans förelåg ej. Inte heller sågs statistisk skillnad i HMGB-1 koncentrationer i synovia beroende på förekomst eller inte av
ledbroskerosion, slipfåror i ledbrosket eller osteokondrala fragment utan kliniska symtom.

Ingen signifikant korrelation mellan nivå av HMGB-1 i serum och synovia kunde hittas, även om koncentrationerna i synovia oftare låg högre än i serum. Materialet var för litet för att kunna undersöka om det fanns en statistisk korrelation mellan grad av ledskada och koncentration av HMGB-1.

Sammanfattningsvis har studien inte kunnat visa att IL-1β eller aktivt MMP-13 kan användas för att diagnostisera patologiska ledförändringar. Däremot att det är möjligt att HMGB-1 kan användas som biomarkör för ledskador med framför allt ett inflammatoriskt inslag. Fortsatta studier behövs för att vidare undersöka potentialen för HMGB-1 som biomarkör för OA.

Lameness is the most common cause of disease in Swedish horses, and the most common cause of lameness is osteoarthritis (OA). Diagnosing OA often comes too late in the disease process so that irreversible damage in the joint has already occurred. Therefore it is crucial to
improve the diagnostic methods in the area. There are several ways in which OA can arise; acute trauma, intraarticular fractures, repetitive trauma and damage to the soft tissue that creates an instability in the joint to name a few. The causes of OA can be summarized as
normal use of an abnormal joint or abnormal use of a normal joint. Regardless of the cause the same type of changes can be seen within the articular cartilage. Depending of the stage of OA radiografic changes can be detected, but these poorly reflect early changes in the cartilage. OA can be considered an inflammatory joint disease where several of the different tissues of the joint contribute to inflammation. For example both the synoviocytes and the
chondrocytes produce inflammatory mediators.

In search for better diagnostic tools for early OA detection, biomarkers may be considered. Such biomarkers could be substances that can be tested for in synovial fluid or serum. In this project the cytokines interleukin (IL)-1β, high mobility group box protein (HMGB)-1 and the
enzyme matrix metalloproteinas (MMP)-13 were investigated. These substances may all be involved in the disease process of OA. The hypotheses of the study were that increased
concentrations of IL-1β, HMGB-1 and active MMP-13 in synovial fluid and serum are associated with the presence and grade of joint lesions. Furthermore, synovial fluid
concentrations of IL-1β, HMGB-1 and active MMP-13 will be correlated to levels in serum.

Fourteen patient horses were included in the study and 24 fetlock and carpal joints were sampled during arthroscopy. Serum and synovial fluid samples were analyzed with specific
ELISAs for IL-1β, HMGB-1 and active MMP-13 concentrations, and pathological changes in joints were recorded and graded during arthroscopy. Leukocyte counts in synovial fluid were
also determined. Active MMP-13 was not detected, and IL-1β was only detected in samples from one horse. HMGB-1 was detected in 20 synovial fluid samples and 12 serum samples,
and concentrations of HMGB-1 in synovial fluid correlated with synovial fluid leukocyte counts. The concentration of HMGB-1 was often higher in samples from joints with presence
of synovitis and fracture but this was not statistically significant. No statistical difference in HMGB-1 concentrations could be shown in synovial fluid in regard to presence of cartilage erosions, wear lines or osteochondral fragments without clinical symptoms. Although there was a tendency for concentrations of HMGB-1 to be higher in synovial fluid than in serum there was no statistical correlation between synovia and serum HMGB-1 concentrations. The graded groups of joint lesions were too small to enable calculations regarding correlations
between grade of lesions and concentrations of HMGB-1.
To conclude, this study has not been able to show that synovial fluid or serum concentrations of IL-1β or active MMP-13 can be used as diagnostic tools to detect joint lesions. However, there is a possibility that synovial fluid concentrations of HMGB-1 can be used as an
indicator of joint lesions, particular those lesions with an inflammatory component. Further studies are required for evaluating HMGB-1 as a biomarker for OA.
