Abstract

Det finns få studier på effekten av och säkerheten med smärtlindring i samband med kejsarsnitt på tik. Av den anledningen är många restriktiva med smärtlindringen. Enligt denna studie var den vanligaste metoden i Sverige att ge en engångsdos buprenorfin när sista valpen
tagits ut genom kejsarsnittet. Internationellt är många mer frikostiga med smärtlindringen. En stor del av litteraturen rekommenderar till exempel preoperativ behandling med opioider, såsom fentanyl, samt smärtlindring med icke-steroida antiinflammatoriska läkemedel (nonsteroidal
anti-inflammatory drugs; NSAIDs) i en eller flera doser.
Examensarbetet gjordes som en del i en sedan tidigare pågående multicenterstudie.

Veterinären valde själv typ av smärtlindring. Frågor om tiken och valparna besvarades av personalen på kliniken i anslutning till kejsarsnittet och av djurägaren hemma den första tiden efter kejsarsnittet. Syftet var att jämföra effekten av vilken smärtlindring som tiken fick, om
någon, på valparnas överlevnad till och med åtta veckors ålder, tikens tillstånd och intresse för valparna till och med en vecka efter kejsarsnittet och valparnas vitalitet direkt efter födseln.

De grupper som jämfördes var tikar som fått enbart opioider, tikar som fått enbart NSAIDs, tikar som fått både opioider och NSAIDs och tikar som inte fått någon smärtlindring alls.

Ingen hänsyn togs till tidpunkten för behandlingen i gruppindelningen. För att få ett tillräckligt stort underlag slogs vid vissa beräkningar grupperna med alla tikar som fått NSAIDs, eller alla tikar som fått respektive inte fått opioider, samman.

Gällande tikarnas allmäntillstånd och intresse för valparna och valparnas vitalitet efter födseln kunde ingen signifikant skillnad mellan grupperna påvisas. Detsamma gällde för överlevnaden bland de levande födda valparna till en vecka och åtta veckors ålder. Vid födseln var andelen döda valpar signifikant lägre i gruppen där tikarna enbart fick opioider än i de andra grupperna. Detta kan dock inte ha haft ett direkt samband med smärtlindringen då
denna i nästan samtliga fall gavs efter att sista valpen tagits ut. Istället kan det ha berott på att vissa tikar fick NSAIDs just för att de inte hade några levande valpar som skulle dia.

Potentiella risker som finns med smärtlindring vid kejsarsnitt på tik är att läkemedlet (som kan tas upp av valparna via placentan eller mjölken) kan ha negativa effekter på valparna i form av till exempel andningsdepression eller störd utveckling, samt i fallet opioider att tiken kan bli sederad eller påverkad så att hon tar sämre hand om sina valpar. Denna studie hade ett
relativt litet urval och vissa tänkbara felkällor, men det kan ändå konstateras att ingen förhöjd risk med administration av smärtlindring (efter att valparna tagits ut) kunde påvisas. Då tillräcklig smärtlindring är viktig, såväl av djurskyddsskäl som för att minska risken att tiken
stöter bort sina valpar och inte släpper ner mjölken, behövs fler studier avseende säkerheten.

Detta gäller inte minst de mer frikostiga internationella rekommendationerna.

There are few studies on the effect and safety of the administration of analgesics for caesarean section in the bitch. For that reason, it is common to be restrictive about this administration.

According to this study, in Sweden it was most common to administer a single dose of buprenorphine after the last puppy was delivered. Internationally, a more liberal
administration of analgesics is common. A large part of the literature does, for example, recommend the pre-operative use of opioids, such as fentanyl, and the use of non-steroidal anti-inflammatory drugs (NSAIDs) as single or repeated doses.

This master´s thesis was done as a part of a multi-centre study that was already ongoing. The veterinarian was free to choose type of analgesia. The staff at the veterinary clinic answered questions about the bitch and puppies at the time of the caesarean section and the owner of the
bitch answered the enquiry at home during the time after surgery. The purpose was to compare the effects of the analgesic given to the bitch, if any, on the survival of the puppies up to eight weeks of age, the well-being of the bitch and her interest in her puppies up to one week after the caesarean section, and the vigour of the puppies immediately after birth. The groups compared were bitches that received opioids only, bitches that received NSAIDs only, bitches that received both opioids and NSAIDs and bitches that didn´t receive any analgesics.

The timing of the treatment was not taken into account when deciding the groups. To make the material big enough for statistical analyses, for some calculations groups were merged so that all bitches receiving NSAIDs made one group, or bitches that did or did not receive opioids made two groups.

No significant difference in the well-being of the bitches or their interest in their puppies, nor in the vigour of the puppies after birth, was found. The same was true about the survival among the puppies born alive up to one week and eight weeks of age. At birth, the relative number of dead puppies was significantly smaller in the group where the bitches received opioids only, than in the other groups. This cannot be directly related to the analgesics
administered, however, since in almost all cases, the drug was not given until the last puppy was delivered. Instead, one reason may be that some bitches were given NSAIDs because of the fact that they had no live puppies that would suckle.

Potential risks with analgesics for caesarean section are that the drug (which is taken up by the puppies through the placenta or milk) may have negative effects on the puppies such as respiratory depression or disturbed development. By the use of opioids, the bitch may also become sedated or affected so that she takes less care of her puppies. In this study, the material was relatively small and there were some possible sources of error, but it can still be
concluded that no increased risk with administering analgesics (after the puppies were delivered) was found. Since it is important to fully relieve pain, as well for the sake of animal welfare as to reduce the risk of the bitch repulsing her puppies or not letting down the milk,
more studies of the safety are needed. This is especially true for the more liberal international recommendations.
