Abstract

Parasiter som utvecklat resistens mot maskmedel (anthelmintika) är ett utbrett problem över hela världen. De preparat som används idag har funnits länge på marknaden och endast ett fåtal nya preparat finns under utveckling. Det är därför angeläget att på sikt finna hållbara
lösningar för att hantera den allt mer växande resistensproblematiken. För detta krävs både utökad kunskap och fortsatt forskning om hur resistens hos parasiter uppkommer och vilka möjligheter som finns att häva den.
Ett flertal teorier kring resistensutveckling hos parasiter kretsar kring effluxproteiner som till exempel P-glykoproteiner (P-gp). En teori är att det hos resistenta parasiter har utvecklats överuttryck av de effluxproteiner som har till uppgift att pumpa ut anthelmintika ur parasiten
varvid effekten av läkemedlet uteblir. Möjligheten att hämma P-gp har rent teoretiskt en följdverkan som innebär ett minskat efflux, vilket i sin tur nedsätter parasitens förmåga att eliminera läkemedlet. Detta resulterar i att koncentrationen av läkemedel i parasiten ökar och
att anthelmintikan får ökad effekt.

I mitt kandidatarbete har jag via litteraturstudier undersökt ABC-transportörernas roll i resistensutvecklingen hos parasiter samt försökt identifiera vilka metoder som idag används i syfte att inhibera effluxproteinernas funktion. I redovisningen har jag valt att koncentrera mig
på den forskning som specifikt rör nematoder.

Jämförelser av olika studier har lett till att jag har dragit slutsatsen att P-gp spelar en betydande roll i resistensutvecklingen mot anthelmintika hos nematoder samt att det tycks finnas möjligheter att öka mottagligheten hos resistenta nematoder genom administration av P-gp inhibitorer i samband med avmaskning. Dock har de flesta substanser med en sådan effekt på P-gp också flertalet biverkningar i form av toxiska effekter på värddjuret.

Resistance agains anthelmintics is a world wide problem, where the drugs we use today are the same as we used 20 years ago and only a few new drugs are beeing developed. Therefore it is of great importance that we can find sustainable methods for the use of anthelmintics so
that the problems we have with resistance today dont get worse. To do this we need more knowledge about how parasites develop anthelmintic resistance and how we can reverse it.

One of the theories concerning the development of anthelmintic resistance is related to efflux proteins like P-gp. According to this theory overexpression of the genes encoding for these proteins causes a higher efflux of the drug from the parasite, wich reduces the effectiveness of
the drug. If there is a possibility to reduce the efflux caused by these proteins the drug concentration in the parasite would rise and also cause a rise of the anthelmintic effectivity.

In this review I have studied possibilities to inhibit these membrane bound transport proteins and a selection of the research made in this field will be presented. I have chosen to focus on nematodes. In my studies I have found that these efflux proteins play a great part in the
development of anthelmintic resistance and that there may be good possibilities to increase the receptiveness in resistant nematodes by co-administration of P-gp inhibitors. Sadly many of the substances with this effect on P-gp also has a great number of toxic effects to the host.
