Abstract

Salmonella kan infektera de flesta av våra husdjur och produktionsdjur och är en av de vanligaste livsmedelsburna infektionerna hos människa. Salmonella smittar fekalt-oralt men vissa serotyper som Salmonella Enteritidis kan även överföras från hönan till nästa generation genom äggen.
I denna litteraturstudie sammanfattas hur Salmonella, särskilt Salmonella Enteritidis, kan få en omfattande och snabb spridning inom hela fjäderfäproduktionen på grund av att bakterien inte bara sprids med avföringen utan även med äggen från infekterade djur. Smittspridningen kan bli särskilt snabb och omfattande om djuren i avelspyramidens topp infekteras. Därigenom kan smitta hos exempelvis några tusen avelsdjur spridas till miljontals kycklingar och ägg i konsumtionsledet med en massiv exponering för konsumenten, människan. Studien innehåller även hur man ska gå till väga för att minimera risken för smittspridningen.
Spridningen av Salmonella Enteritidis ökade kraftigt under åren 1979-1987 och utvecklade sig till en global pandemi av Salmonellainfektion hos människor. Orsaken var att det då uppträdde en tidigare inte observerad stam av Salmonella Enteritidis som kunde infektera hönans ägg. Tidigare hade alltid det inre av äggen betraktas som sterilt och säkert att konsumera. För att stoppa smittspridningen har det visats vara viktigt att eliminera smittan särskilt i avelspyramidens topp. Genom en sådan strategi har antalet årliga salmonellafall på människor halverats inom EU. Sverige hade redan ett kontrollprogram på plats varigenom den globala spridningen inte nådde den svenska fjäderfäproduktionen av slaktkyckling och konsumtionsägg.

Salmonella can infect most of our pets and farm animals and is one of the most common food-borne infections in humans. Salmonella is transmitted by the fecal-oral route, but some serotypes of Salmonella Enteritidis can also be transmitted from the hen to the next generation through the eggs.
This study summarizes how Salmonella, especially Salmonella Enteritidis, can have a substantial and rapid spread throughout the poultry production because the bacterium is not only spread by feces, but also with the eggs from infected animals. The spread of infection can be particularly rapid and extensive if the top of the breeding pyramid is infected. Then for example a few thousand breeding animals can spread the infection to millions of chickens and eggs at the consumption level resulting in a massive exposure to consumers, people. The study also includes how to proceed in order to minimize the risk for spreading the infection.
The spread of Salmonella Enteritidis increased sharply in different parts of the world during the years 1979-1987, and developed into a true pandemic of Salmonella infection in humans as well as in animals. The reason was that a previously not observed strain of Salmonella Enteritidis occurred that could infect the hen's egg. Previously always the interior of the eggs was considered sterile and safe to consume. In order to stop the spread of infection it has been shown to be important to eliminate the infection particularly in the top of the breeding pyramid. By in the implementation of this approach, the annual number of Salmonella cases in humans decreased with half in EU. Sweden already had a surveillance monitoring program in place, whereby the global spread did not reach the Swedish poultry production.
