Abstract

Meticillinresistenta Staphylococcus aureus är ett växande problem inom hästmedicinen och orsakar svårbehandlade infektioner världen över. Resistensen utvecklas genom att stafylokocker erhåller en gen, mecA, som ändrar β-laktamantibiotikas målstruktur. Idag är MRSA-utbrott vanligt i många länder, men hittills har vi i Sverige inte konstaterat mer än 20 fall på häst. Risken för att fler fall kommer att upptäckas i framtiden är dock stor på grund av import av hästar och smittspridning via människor.
Ett oerhört viktigt moment vid misstanke om MRSA-infektion är var man ska provta och hur man bör odla, typa och namnge de olika isolaten. Idag finns ingen väldefinierad standard eller några riktlinjer att följa på dessa punkter vilket orsakar problem vid jämförelser av isolat samt försvårar smittspårning. Det finns många olika odlingsmedier och typningsmetoder att tillgå, alla med sina fördelar och nackdelar. Pulsed-field gel electrophoresis (PFGE) har i flera år varit den mest använda metoden vid typning av isolat men har mer och mer börjat ersättas av bland annat spa-typning och multi-locus sequence typing (MLST). En grundlig utvärdering och standardisering av de olika metoderna skulle vara av stor nytta för internationell smittspårning och isolatjämförelse samt ge kunskap om hur man på bästa sätt kan förhindra spridning

Meticillin resistant Staphylococcus aureus is a growing concern worldwide in equine medicine. It mostly causes wound infections that are very difficult to treat. The antibiotic resistance of the staphylococci is developed by the acquisition of a gene, mecA, which changes the target structure for the β-lactam antibiotics. Today MRSA outbreaks are seen regularly in many countries, but so far Sweden has only identified 20 equine cases. The risk of finding more cases in the future is high due to importation of horses and spread of infection by humans.

A critically important question when MRSA infection is suspected is where to sample and how to culture, type and name the isolates. Today there are no well-defined standard or guidelines for this, which causes problems when it comes to comparing isolates and tracing the source of infection. There are many different culture media and typing methods available, all with advantages and disadvantages. Pulsed-field gel electrophoresis (PFGE) has been the most commonly used typing method but is now increasingly being replaced by methods such as spa-typing and multi-locus sequence typing (MLST). A thorough evaluation and standardization of the different methods would be of great benefit to the international tracing of the infection sources, comparison of isolates and in providing information on how to effectively hinder spread of the bacteria.
