Abstract

Vibrio vulnificus är en halofil bakterie, som framförallt har hittats längs med Golfkusten i USA och i flera asiatiska länder. Den kan orsaka gastroenterit, sepsis eller sårinfektioner och medför en hög mortalitet. I USA står den för en stor del av de skaldjursassocierade dödsfallen. Såväl ostron, fisk och musslor som sediment, vatten och plankton har i olika studier beskrivits som reservoarer för V. vulnificus. Det finns även en teori om att V. vulnificus kan överleva i tarmen hos skarvar och spridas ut i miljön via faeces.
Man kan infekteras via konsumtion eller hantering av skaldjur eller via sår som kommer i kontakt med havsvatten. Personer med förhöjda järnnivåer, leversjukdomar eller olika immunbrister samt äldre personer löper en större risk att drabbas. V. vulnificus dör vid upphettning samt frysning och är känslig för flera olika antibiotika, men även kirurgi kan krävas för att bli av med bakterien. Det är ovanligt med fall av V. vulnificus i Europa, men under sommaren 2006 hade Sverige ett antal fall av bakterien och sjukdomsfall har även förekommit i bland annat Danmark och Israel.
Varmare vattentemperaturer till följd av de förutspådda klimatförändringarna ökar risken för V. vulnificus och de fall av V. vulnificus som har rapporterats i Europa har skett under ovanligt varma somrar. Studier har visat att V. vulnificus trivs bäst vid 14-26oC, vilket Östersjön med de förväntade klimatförändringarna kommer att uppfylla under somrarna. Den lägsta salthalten där V. vulnificus tillväxer ligger runt 5 ‰, vilket i stort sett allt vatten runt Sverige uppfyller. Befolkningen i Sverige blir allt äldre, personer med immunbrister och leversjukdomar lever allt längre och skaldjurskonsumtionen ökar. I och med allt detta kan V. vulnificus mycket väl komma att bli ett problem i Sverige och för att minska antalet fall behöver medborgarna informeras om riskerna med att äta ostron och att bada i havet med öppna sår.

Vibrio vulnificus is a halophilic bacterium, which is mainly found along the Gulf Coast in the United States and in many Asian countries. It may cause gastroenteritis, sepsis or wound infections and has a high mortality. The bacterium causes many of the seafood-related deaths in the United States and it has several reservoirs, including oysters, fish, mussels, sediment, water and plankton. There is also a theory that V. vulnificus may survive in the intestines of cormorants and get into the environment through faeces.
One may get infected by consuming or handling seafood, but also if wounds come in contact with sea-water. People with elevated iron levels, liver diseases or different immunodeficiencies as well as old people have a greater risk of getting infected. V. vulnificus dies if heat treated or frozen and it is sensitive to many antibiotics, but even surgery might be necessary to get rid of the bacterium. Cases of V. vulnificus are rare in Europe, but during the summer 2006 there were some cases in Sweden, and there have also been some cases in for instance Denmark and Israel.
Higher water temperatures as a result of the predicted future climate changes will increase the risk of V. vulnificus and the reported cases in Europe emerged during unusually warm summers. Studies have shown that the optimal water temperature for V. vulnificus is 14-26oC, which the Baltic Sea will reach during the summers if the predicted climate changes occur. The lowest salinity for V. vulnificus to grow is around 5 ‰, which is more or less what all the water around Sweden achieves. The Swedish population gets older, people with immunodeficiencies and liver diseases live longer and the seafood consumption increases. All in all this indicates that V. vulnificus will become a future problem in Sweden and to lower the number of cases it is important to inform the citizens about the risks of eating oysters and swimming in the sea with open wounds.
