Abstract

Ekvint metabolt syndrom (EMS) är ett sjukdomskomplex som karaktäriseras av en ökad fettansättning, generell eller regional, insulinresistens och predisposition att utveckla fång. Individer som drabbas är ofta medelålders till äldre och ofta av ponnyras. Idag syftar behandlingen av hästar med EMS till att minska deras insulinresistens, vilket kan göras genom att få hästarna att minska i vikt, dels genom förändrad diet med ett restriktivt energiinnehåll, dels genom ökad motion. För att förhindra att dessa hästar får fång är det viktigt att kunna identifiera högriskindivider samt att behandla hästar med EMS innan de drabbas av följdsjukdomar. Syftet med studien var att undersöka hur insulinresistensen hos hästar som är diagnosticerade med EMS påverkas av behandling med viktreduktion. Ett annat syfte var att hitta kliniskt användbara parametrar för att identifiera viktminskning.
Hästarna i studien var privatägda. De har genom oralt glukostoleranstest (OGT) utfört i fält konstaterats insulinresistenta. Andra orsaker till insulinresistens så som hypofysär pars intermedia dysfunktion (PPID) har uteslutits genom analys av ACTH-nivåer. Tjugo hästar av olika ras, varav 16 ston och 4 valacker stallades upp på hästkliniken vid Universitetsdjursjukhuset, Uppsala. Där utfördes OGT och euglykemisk hyperinsulinemisk clamp (EHC). Vid EHC användes glukosflödet vid steady state för att kalkylera insulinkänsligheten hos hästarna. Värdet för insulinkänsligheten användes sedan för att beräkna den förväntade insulinpeaken och betacellsresponsen. Hästarna bedömdes avseende deras hull samt regionala fettansättningar. Objektiva mått togs av nacke, mankhöjd, bröst- och bukomfång. Hästarna fick efter provtagningarna återvända hem till sina privata stallar. De fodrades med en foderstat anpassad för viktreduktion med ett lågt innehåll av lättlösliga kolhydrater. Efter 10 till 16 månader gjordes en uppföljning av hästarnas värden och mått. Behandlingseffekten utvärderas genom att hästarna i studien utgjorde sina egna kontroller.
Totalt 11 av 20 hästar fullföljde studien. Nio av dessa hade en sjukdomshistorik som involverade fång. Efter första undersökningen analyserades fodret hos 9 av hästarna som därmed stod på en uträknad foderstat för viktkorrigering och som hade ett känt värde av lättlösliga kolhydrater. Nio hästar hade minskat i vikt vid den uppföljande undersökningen. Hästarnas totala viktförändring visade i medeltal en minskning med 20,2 ± 21,4 kg (p= 0,01) mellan undersökningarna. Åtta hästar hade förbättrat sin insulinkänslighet. Sammantaget hade hästarna i studien ökat sin insulinkänslighet med i medeltal 0,4 ± 0,6 mg/kg/min (p= 0,03). Ett hyperbolt samband identifierades mellan insulinkänsligheten och insulinpeak samt betacellsrespons. Minskningen i vikt var associerad till den förbättrade insulinkänsligheten (r2=0.45, p=0,02) och till en lägre bedömning av body condition score (BCS) (r2= 0,67, p= 0,002), men inte till cresty neck score (CNS) (r2= 0,35, p=0,056). De objektiva mätvärdena korrelerade till viktminskningen med ett minskat bröstomfång (r2= 0,45, p = 0,02), bukomfång (r2 = 0,54, p = 0,01), medelvärdet av nackens omkrets vid 3 olika punkter (r2= 0,69, p=20,002), samt en minskad kvot mellan nackens omkrets mitt på halsen och hästens mankhöjd (r = 0,49, p = 0,02). Förändringen i insulinkänslighet var korrelerad förutom till vikten, även till skillnaden i CNS (r2=0,57, p=0,007), samt till kvoten mellan omkretsen mitt på halsen och hästens mankhöjd (r2= 0,51, p = 0,01).
Resultatet indikerar att det finns ett förhållande mellan viktminskning och en förbättrad insulinkänslighet hos hästar med EMS. Dock är inte den ökade insulinkänsligheten, som erhålls av viktminskningen hos hästar med EMS, tillräcklig för att de ska anses vara friska. Däremot kommer en liten förbättring av insulinkänsligheten leda till en avsevärt mindre postprandiell insulinrespons, vilket minskar risken för att utveckla fång. Studien tyder på att BCS och objektiva mått tagna av olika delar på hästens kropp kan vara en värdefull hjälp för att följa hästens viktminskning i samband med bantning och dietära åtgärder hos hästar med EMS.

Equine metabolic syndrome (EMS) is defined as obesity, general or regional, insulin resistance and a predisposition to develop laminitis. It often affects ponies in middle age or older ponies. Treatment for EMS is aimed to increase insulin sensitivity. This can be achieved by weight loss through a diet with restricted energy intake combined with physical exercise. It’s important to identify individuals at risk at an early stage and to treat horses with EMS in order to prevent the development of laminitis. The purpose of this study was to investigate how weight reduction affects insulin resistance in horses diagnosed with EMS. A second aim was to identify parameters for weight reduction and can be used in a field situation where a scale is not available.
The horses in this study were privately owned. They had previously been diagnosed with EMS by an oral glucose tolerance test (OGT). Other reasons for insulin resistance such as pituitary pars intermedia dysfunction (PPID) were ruled out by analysis of ACTH levels. In total 20 horses of different breeds, 16 mares and 4 geldings, were included in the study. The horses were housed at the Equine Clinic at the Swedish University of Agricultural Sciences in Uppsala, where both an OGT and an euglycemic hyperinsulinemic clamp (EHC) were performed. The rate of the glucose infusion during steady state of the EHC was used to estimate the insulin sensitivity (M value) of the horses. Using the disposition index, the horses presumed insulinpeak and beta cell response from an OGT were calculated from the M values. Body condition and regional fat deposits were evaluated in each horse. The horses’ neck, chest, abdomen and height were measured. After the sampling procedures, the horses returned to their home environment. They were fed a diet calculated to lose weight and with a low content of water soluble carbohydrates (WSC). The horses were brought back after 10 to 16 months to repeat the procedures.
In total 11 out of 20 horses participated throughout the study. Nine of the horses had a medical history involving laminitis. Nine horses were fed a diet with restricted intake of energy and with a known content of WSC. At the time for the second evaluation 9 horses had lost weight. In total all horses had decreased their weight with a mean of 20,2 ± 21,4 kg (p= 0,01). Eight had improved their insulin sensitivity, the change in insulin sensitivity considered all horses had a mean of 0,4 ± 0,6 mg/kg/min (p= 0,03). The weight loss was correlated with the improvement of insulin sensitivity (r2= 0.45, p= 0,02) and with a decrease in Body Condition Score (BCS) (r2 = 0,67, p= 0,002), but not with a decrease in Cresty Neck Score (CNS) (r2= 0,35, p= 0,056). The weight loss was correlated with the decreased circumference of the thorax (r2 =0,45, p= 0,02), abdomen (r2 = 0,54, p= 0,01), and the neck (r2 = 0,69, p= 0,002) as well as with the ratio between the measurement of the neck and height (r2 = 0,49, p= 0,02). The increased insulin sensitivity was correlated with the decrease in CNS (r2= 0,57, p= 0,007) as well as with the ratio between the neck and height (r2= 0,51, p= 0,01).
This study indicates that horses with EMS can improve their insulin sensitivity by weight loss. However, the degree of weight loss was not sufficient to obtain normal insulin sensitivity. Nevertheless, because of the non-linear inverse relationship between insulin sensitivity and the beta cell response, a small increase in insulin sensitivity will result in a much lesser postprandial insulin secretion and reduce the risk of developing laminitis. The study also reveals that BCS and other objective measurements of the neck and abdomen can be used to assess weight loss in the field.
